摘要
目的:探究类风湿关节炎(RA)患者血清14-3-3η蛋白及其mRNA表达水平与关节损伤的关系。方法:选择2017年1月至2018年12月收治的RA患者158例,其中早期RA 80例,非早期RA 78例,另招募同期体检健康者50例为对照组;采用酶联免疫吸附法(ELISA)检测血清14-3-3η蛋白,C反应蛋白(CRP)、类风湿因子(RF)、抗环瓜氨酸抗体(抗CCP抗体)等浓度,实时荧光定量-聚合酶链式反应法(qRT-PCR)检测血清14-3-3ηmRNA浓度,应用RAMRIS系统评分、28个关节肿胀关节个数、28个关节压痛关节个数及28个关节疾病活动度评分评价关节损伤表现,分析血清14-3-3η蛋白及其mRNA表达水平与关节损伤的关系。结果:RA患者血清14-3-3η蛋白及mRNA、ESR、CRP、RF和抗CCP抗体水平均显著高于正常对照组(均P<0.05);当14-3-3η蛋白浓度≥1.37 ng/ml时,ROC曲线下面积(AUC)为0.824,灵敏度为84.00%,特异度为81.00%,诊断价值最高;非早期RA组14-3-3η蛋白及mRNA表达水平显著高于早期RA组(均P<0.05);RA患者DAS28评分为(7.58±1.58)分,28个关节肿胀关节(13.49±3.42)个,28个关节压痛关节(12.73±3.16)个,RA患者滑膜炎评分为(1.75±0.86)分,骨髓水肿评分为(1.48±0.51)分,骨侵蚀评分为(3.26±1.43)分;RA患者血清14-3-3η蛋白与CRP水平、滑膜炎评分、骨髓水肿评分、DAS28评分、28个关节肿胀关节个数及压痛关节个数呈正相关(r/P=0.462/0.039,0.427/0.045,0.384/0.017,0.528/0.032,0.451/0.008,0.419/0.027)。结论:血清14-3-3η蛋白及其mRNA表达水平在内风湿关节炎患者中明显升高,且与关节损伤密切相关。
Objective:To investigate the relationship between expression levels of serum 14-3-3η protein,mRNA and joint injuryin patients with rheumatoid arthritis(RA).Methods:A total of 158 RA patients who were admitted from January 2017 to December 2018 were enrolled,including 80 cases with early RA,78 cases with non-early RA.50 healthy people undergoing physical examination in the same period were enrolled as control group.The enzyme-linked immunosorbent assay(ELISA) was performed to detect concentrations of serum 14-3-3η protein,C reactive protein(CRP),rheumatoid factor(RF) and anti-cyclic citrullinated peptides antibody(anti-CCP antibody).Quantitative real-time PCR(qRT-PCR) was performed to detect concentration of serum 14-3-3η mRNA.RAMRIS system scoring,joint number of 28 joints swelling,joint number of 28 joints tenderness and disease motion of 28 joints were performed to evaluate findings of joint injury.The relationship between expression levels of serum 14-3-3η protein,mRNA and joint injury was analyzed.Results:The levels of serum 14-3-3η protein and mRNA,ESR,CRP,RF and anti-CCP antibody in RA patients were significantly higher than those in normal control group(P<0.05).When 14-3-3η protein concentration was not less than 1.37 ng/ml,the area under the receiver operating characteristic(ROC) curve(AUC),sensitivity and specificity were 0.824,84.00% and 81.00%,respectively.And the diagnostic value was the highest.The expression levels of 14-3-3η protein and mRNA in non-early RA group were significantly higher than those in early RA group(P<0.05).DAS28 score,joint number of 28 joints swelling and joint number of 28 joints tenderness in RA patients were(7.58±1.58) points,(13.49±3.42) and(12.73±3.16),respectively.The scores of synovitis,bone marrow edema and bone erosion in RA patients were(1.75±0.86) points,(1.48±0.51) points and(3.26±1.43) points,respectively.The serum 14-3-3η protein of RA patients was positively correlated with CRP level,synovitis score,bone marrow edema score,DAS28 score,joint number of 28 joints swelling and joint number of 28 joints tenderness(r/P=0.462/0.039,0.427/0.045,0.384/0.017,0.528/0.032,0.451/0.008,0.419/0.027).Conclusion:The expression levels of serum 14-3-3η protein and mRNA are significantly increased in RA patients,which are closely related to joint injury.
