摘要
目的探讨5A+5R干预联合伐尼克兰对慢性阻塞性肺疾病(COPD)患者戒烟效果及肺功能的影响。方法将2017年1月~2018年6月收治的120例COPD患者随机分为2组,对照组(n=60)给予伐尼克兰口服,干预组(n=60)在此基础上给予5A+5R策略进行戒烟干预,共持续12周。比较两组患者随访1、3、6个月时的戒烟率,干预前后尼古丁依赖检测量表(FTND)评分、肺功能及生活质量变化。结果随访3、6个月时,干预组戒烟成功率显著高于对照组,FTND评分亦明显升高(P<0.05)。随访6个月,两组各肺功能指标均不同程度改善,干预组FVC、FEV1、FEV1/FVC水平均显著优于对照组(P<0.05)。随访6个月,干预组呼吸症状、活动受限及疾病影响评分较干预前明显下降,且明显低于对照组(P<0.05)。结论 5A+5R干预联合伐尼克兰可提高具有烟草依赖性的COPD患者戒烟成功率,改善肺功能,提升生活质量。
Objective To explore the effect of 5 A+5 R intervention combined with varnicrane on smoking cessation and pulmonary function in patients with COPD. Methods 120 patients with COPD admitted from January 2017 to June 2018 were randomly divided into two groups. The control group(n=60) was given the oral administration of varnicrane, and the intervention group(n=60) was given the smoking cessation intervention with 5 A+5 R strategy, which both lasted for 12 weeks. The smoking cessation rates at follow-up of 1, 3 and 6 months, nicotine dependence test scale(FTND) scores, lung function and quality of life before and after intervention were compared between the two groups. Results At the follow-up of 3 and 6 months, the success rate of smoking cessation in the intervention group was significantly higher than that in the control group, and the FTND score was also significantly increased(P<0.05). Following up for 6 months, all pulmonary function indicators in the two groups were improved at different degrees. The levels of FVC, FEV1 and FEV1/FVC in the intervention group were significantly better than those in the control group(P<0.05). Following up for 6 months, respiratory symptoms, activity restriction and disease influence scores of SGRQ in the intervention group were significantly lower than those before intervention, and they were significantly lower than those in the control group(P<0.05). Conclusion 5 A+5 R intervention combined with varnicrane can improve the success rate of smoking cessation, improve pulmonary function and improve the quality of life in patients with COPD.
