摘要
目的:观察高压氧联合负压封闭引流(VSD)治疗感染性糖尿病足溃疡的近期临床疗效。方法:将感染性糖尿病足溃疡患者50例,随机分为两组,对照组25例在常规内科治疗基础上予VSD治疗,高压氧组25例在对照组基础上进行高压氧治疗,观测两组治疗前及治疗第24天后,即高压氧治疗20次后的创面面积、视觉模拟评分法(VAS)评分,白细胞计数(WBC)、C-反应蛋白(CRP)、体温,血脂,血液流变学指标的变化情况。结果:两组患者的创面面积、WBC、CRP较治疗前都有改善,高压氧组优于对照组,差异有统计学意义(P<0.05),VAS评分、体温改善不明显,无统计学意义(P>0.05)。两组血脂水平均较治疗前改善,组间差异无统计学意义(P>0.05);两组血液流变学均较治疗前有改善,高压氧组优于对照组,差异有统计学意义(P<0.05)。结论:高压氧联合负压封闭引流可以有效改善糖尿病足溃疡下肢缺氧、控制感染,加速创面愈合,值得推广应用。
Objective To observe the recent therapeutic effect of diabetic foot ulcer treated with hyperbaric oxygen therapy combined with vacuum sealing drainage.Method A total of 50 patients with diabetic foot ulcer were randomly allocated to an hyperbaric oxygen group(hyperbaric oxygen therapy combined with vacuum sealing drainage,n=25) and a control group(Medical therapy combined with vacuum sealing drainage,n=25).To observe the indicator before the treatment and 24 days after the treatment(after hyperbaric oxygen therapy for 20 times),the observed indicator including Wound area,VAS,WBC,CRP,body temperature,serum lipid and Hemorheology.Results The wound area,WBC,CRP and Hemorheology in both groups were improved after treatment.The hyperbaric oxygen group showed statistically significant improvements to the control group(P<0.05),showed no statistically significant improvements in VAS,body temperature and serum lipid to the control group(P>0.05).The level of blood lipid in the two groups was better than that before treatment, the difference was not statistically significant(P>0.05). The hemorheology of the two groups was improved, The hyperbaric oxygen group was better than the control group, the difference was statistically significant(P<0.05). Conclusion Hyperbaric oxygen therapy combined with vacuum sealing drainage therapy for diabetic foot ulcer can effectively improve the symptoms of lower limbs of hypoxia、control infection、Promoting wound Healing.It is worthy of popularization and application.
