空腹血糖受损人群炎症标志物与胰岛素抵抗、胰岛β细胞功能受损的相关性分析 被引量:10

摘要 目的:探讨空腹血糖受损(IFG)人群的炎症标志物与胰岛素抵抗(IR)、胰岛β细胞功能的相关性。方法:以IFG组为研究对象,2型糖尿病组(T2DM)和健康正常组(HC)为对照,分析IFG人群炎症指标如超敏C反应蛋白(Hs-CRP)、血沉(ESR)、中性粒细胞/淋巴细胞比值(NLR)、单核细胞/淋巴细胞比值(MLR)和血小板/淋巴细胞比值(PLR)与糖代谢指标[空腹血糖(FPG)和糖化血红蛋白(HbA1c)]、胰岛素代谢指标[空腹胰岛素(FINS)、稳态模型评估胰岛素抵抗指数(HOMA-IR)和稳态模型评估胰岛β细胞功能指数(HOMA-β)]等的相关性。结果:3组炎症指标Hs-CRP、NLR和ESR之间有差异(均P<0.05);IFG组Hs-CRP与FPG、HbA1c、FINS、HOMA-IR均呈正相关(r分别为0.330、0.270、0.387、0.330,均P<0.05),与HOMA-β呈负相关(r为-0.387,P<0.05));NLR与FPG、HbA1c、HOMA-IR均呈正相关(r分别为0.218、0.363、0.336,均P<0.05),与HOMA-β呈负相关(r为-0.289,P<0.05)。结论:IFG人群的炎症标志物Hs-CRP、NLR与胰岛素抵抗、胰岛β细胞功能受损具有相关性。 Objective:To investigate the relationship between inflammatory markers,insulin resistance(IR)and isletβ-cell function in patients with impaired fasting glucose(IFG).Methods:The IFG group was used as the study group,type 2 diabetes group(T2DM)and healthy control(HC)were used as the control groups.The correlation between inflammatory markers[high-sensitivity C-reactive protein(Hs-CRP),erythrocyte sedimentation rate(ESR),neutrophil/lymphocyte ratio(NLR),monocyte/lymphocyte ratio(MLR)and platelet/lymphocyte ratio(PLR)]and glucose metabolism indicators[fasting blood glucose(FPG),glycated hemoglobin(HbA1c)]and insulin metabolism indicators[fasting insulin(FINS),homeostatic model assessment of insulin resistance(HOMA-IR),homeostasis modelassessment-βcell function(HOMA-β)]were analyzed.Results:There were significant difference in Hs-CRP,NLR and ESR between three groups(all P<0.05).Hs-CRP was positively correlated with FPG,HbA1c,FINS and HOMA-IR in IFG group(r=0.330,0.270,0.387,0.330,all P<0.05,respectively),while negatively correlated with HOMA-β(r=-0.387,P<0.05).NLR was positively correlated with FPG,HbA1c and HOMA-IR(r=0.218,0.363,0.336,all P<0.05),but negatively correlated with HOMA-β(r=-0.289,P<0.05).Conclusion:The inflammatory markers Hs-CRP and NLR in people with IFG are associated with insulin resistance and impaired isletβcell function.
