摘要
目的利用Meta分析评价针灸治疗产后缺乳的临床疗效。方法计算机检索中文数据库:中国知网(CNKI),万方数据库,重庆维普全文数据库,中国生物医学文献数据库(CBM)及英文数据库:PubMed,EMbase,Web of Science关于针灸治疗产后缺乳的随机对照试验,检索日期自建库到2018年7月16日。使用Revman 5.3及Stata 12进行Meta分析。结果共纳入文献19篇,2385名产后缺乳患者。Meta分析结果显示:治疗组与对照组比较,总有效率[OR=4.89,95%CI(3.67,6.51),P<0.00001]、治愈率[OR=3.36,95%CI(1.84,6.14),P<0.0001]、泌乳评分[MD=0.42,95%CI(0.25,0.58),P<0.00001]、乳房充盈评分[MD=0.58,95%CI(0.46,0.70),P<0.00001]差异均有统计学意义,泌乳素[MD=60.61,95%CI(-4.49,125.72)]差异无统计学意义(P=0.07)。结论针灸治疗产后缺乳疗效肯定,但因纳入文献质量不高,有待高质量、大样本的临床随机对照试验验证。
Objective To evaluate the clinical effect of acupuncture and moxibustion therapies for the postpartum hypogalactia by meta-analysis.Methods Randomized cont-rolled trails of acupuncture and moxibustion in the treatment of postpartum hypogala-cita were searched from Databases of China National Knowledge Infrastructure(CN-KI),WangFang,Chinese Scientific and Technological Journals(VIP),China Biomedicine(CBM),PubMed,EMbase and Web of Science.The meta-analysis was carried out using software Revman 5.3 and Stata 12.Results:A total of 19 studies involing 2385 patients were included.Results Of meta-analysis showed significant differences between acupuncture-moxibustion intervention group and control group in terms of total effective rate[OR=4.89,95%CI(3.67,6.51),P<0.00001],cure rate[OR:3.36,95%CI(1.84,6.14),P<0.0001],milk secreted score[MD=0.46,95%CI(0.25,0.58),P<0.00001]and degree of mammary fullness[MD=0.58,95%CI(0.46,0.70),P<0.00001]but no difference in prolactin[MD=60.61,95%CI(-4.49,125.72),P=0.07].Conclusion Acupuncture-moxibustion is effective in the treatment of postpartum hypogalactia.However the qualities of these included studies are low,the conclusion needs to be confirmed by high-quality RCTs.
