摘要
目的 探讨皮肤镜在诊断基底细胞癌中的应用价值.方法 收集临床上拟诊为基底细胞癌的病例.通过临床经验丰富的皮肤科医师根据病史及皮损的临床表现进行临床诊断,然后采用皮肤镜诊断,最后以组织病理学检查为"金标准",研究皮肤镜诊断基底细胞癌的敏感性、特异度及一致性.结果 与病理诊断相比较,A、B两位医生使用皮肤镜诊断基底细胞癌一致率分别为90.67%、90.67%、灵敏度分别为88.10%、90.48%、特异度分别为93.94%、90.91%、误诊率分别为6.06%、9.09%、漏诊率分别为11.90%、9.52%、Youden指数分别为82.04%、81.39%、Kappa值分别为0.8125、0.8113.结论 皮肤镜对基底细胞癌诊断与组织病理学检查结果有较好的一致性,具有较高的价值,值得临床推广应用.
Objective To explore the application value of dermoscopy in the diagnosis of basal cell carcinoma(BCC).Methods Clinical proposed cases of BCC were collected.The sensitivity,specificity and consistency of the diagnosis of BCC by dermoscopy were studied through clinical diagnosis by dermatologists with rich clinical experience according to medical history and clinical manifestations of skin lesions,and then dermoscopy was used for diagnosis,and finally histopathological examination was taken as the"golden standard".Results Compared with those of pathological diagnosis,the data of doctor A and doctor B's diagnosis of BCC by dermoscopy were as follows:the coincidence rate(90.67%,90.67%),sensitivity(88.10%,90.48%),specificity(93.94%,90.91%),misdiagnosis rate(6.06%,9.09%),missed diagnosis rate(11.90%,9.52%),Youden index(82.04%,81.39%),Kappa value(0.8125,0.8113)respectively.Conclusion The diagnosis of BCC by dermoscopy has a good consistency with the result of histopathological examination.It is of high value and worthy of clinical promotion and application.
