2型糖尿病患者血清和尿颗粒蛋白前体的水平与其炎性指标及肾功能指标的相关性

摘要 目的:分析2型糖尿病(T2DM)患者血清和尿颗粒蛋白前体(PGRN)的水平与其炎性指标及肾功能指标的相关性。方法:根据肾小球滤过率(eGFR)的不同,将某医院收治的88例T2DM患者及在该院进行体检的30例健康人分为1组、2组、3组、4组。对四组受检者均进行血清PGRN、血清超敏C反应蛋白(hs-CRP)、血清白介素6(IL-6)及尿白蛋白(Alb)、尿PGRN检测,比较其检测结果。结果:1组患者尿Alb及血清PGRN、hs-CRP、IL-6的水平均高于2组患者、3组患者和4组受检者,P<0.05;其eGFR的水平低于3组患者、2组患者和4组受检者,P<0.05。2组患者尿PGRN的水平高于1组患者、3组患者和4组受检者,P<0.05。T2DM患者血清PGRN的水平与其尿白蛋白、血清hsCRP、血清IL-6的水平均呈正相关,该病患者血清PGRN的水平与其eGFR呈负相关。T2DM患者尿PGRN的水平与其eGFR呈正相关,该病患者尿PGRN的水平与其尿白蛋白、血清hsCRP、血清IL-6的水平均呈负相关。结论:T2DM患者并发肾功能不全后其血清PGRN的水平可升高,尿PGRN的水平可降低。T2DM患者血清PGRN的水平与尿白蛋白、血清hsCRP、血清IL-6的水平均呈正相关,与eGFR呈负相关。T2DM患者尿PGRN的水平与eGFR呈正相关,与尿白蛋白、血清hsCRP、血清IL-6的水平均呈负相关。 objective:to analyze the correlation between serum and urine granular protein precursor(PGRN)levels in patients with type 2 diabetes mellitus(T2DM)and their inflammatory and renal indexes.Methods:according to the difference of glomerular filtration rate(eGFR),88 patients with T2DM admitted to a hospital and 30 healthy people who underwent physical examination in the hospital were divided into 1 group,2 group,3 group and 4 group.Serum PGRN,serum hypersensitive c-reactive protein(hs-crp),serum interleukin-6(il-6),urinary albumin(Alb)and urinary PGRN were detected in all four groups,and the results were compared.Results:urinary Alb,serum PGRN,hs-crp and il-6 levels of patients in group 1 were higher than those of patients in group 2,group 3 and group 4(P<0.05).The level of eGFR was lower than that of 3 groups of patients,2 groups of patients and 4 groups of patients(P<0.05).The urine PGRN level of patients in the 2 groups was higher than that of patients in the 1 group,patients in the 3 groups and patients in the 4 groups(P<0.05).The serum PGRN level of T2DM patients was positively correlated with the levels of urinary albumin,serum hsCRP and serum il-6,while the serum PGRN level of T2DM patients was negatively correlated with eGFR.The urine PGRN level of T2DM patients was positively correlated with its eGFR,while the urine PGRN level of T2DM patients was negatively correlated with their urine albumin,serum hsCRP and serum il-6 levels.Conclusion:serum PGRN level can be increased and urine PGRN level can be decreased in T2DM patients with renal insufficiency.Serum PGRN level in T2DM patients was positively correlated with the levels of urinary albumin,serum hsCRP and serum il-6,and negatively correlated with eGFR.Urine PGRN level in T2DM patients was positively correlated with eGFR,and negatively correlated with urine albumin,serum hsCRP and serum il-6 le vels.
