摘要
目的探讨助产责任制护理联合产时体位管理在单胎头位初产妇分娩中的应用效果.方法将980例单胎头位初产妇按随机数字表法分为两组,每组490例,观察组采用助产责任制护理联合产时体位管理,对照组采用助产轮班制护理.统计两组产妇分娩方式、疼痛程度等围生期指标,产程时间、产后出血量等综合性指标,分娩结局及产妇并发症发生状况.结果观察组在第一产程潜伏期和活跃期Ⅱ级/Ⅲ级疼痛发生率显著低于对照组(P<0.01),自然分娩率显著高于对照组(P<0.01),产后出血量显著少于对照组(P<0.01),总产程显著短于对照组(P<0.01).观察组产后下肢麻木、产后出血、产程异常发生率显著低于对照组(P<0.05或0.01),新生儿酸中毒、胎儿窘迫、新生儿窒息发生率显著低于对照组(P<0.05或0.01).结论助产责任制护理联合产时体位管理可以有效减轻单胎头位初产妇第一产程疼痛感,缩短产程,提高自然分娩率,减少产后出血量,降低并发症发生率,改善分娩结局.
Objective To analyze the application of midwifery responsibility system nursing combined with postural management in the delivery of single-head primiparous women.Methods 980 cases of single head primes were selected and randomly divided into two groups,490 cases each.The observation group adopted midwifery position management with midwifery responsibility nursing,and the control group adopted midwifery shift nursing.The perinatal indicators such as maternal delivery,pain,and comprehensive indicators such as duration of delivery,postpartum hemorrhage,delivery outcomes,and maternal complications in the two groups were counted.Results The incidence rate of gradeⅡ/gradeⅢpain in the incubation period and active period of the first stage of labor was significantly lower in the observation group than in the control group(P<0.01).The rate of spontaneous delivery was significantly higher than that of the control group(P<0.01),and the postpartum hemorrhage was significantly less than that of the control group(P<0.01),and the total delivery time was significantly shorter than that of the control group(P<0.01).The incidence of postpartum lower limb numbness,postpartum hemorrhage,and labor abnormalities in the observation group were significantly lower than those in the control group(P<0.05 or 0.01).The incidence of neonatal acidosis,fetal distress,and neonatal asphyxia were significantly lower than those in the control group(P<0.05 or 0.01).Conclusions The combined implementation of midwifery responsibility system nursing and delivery position management can effectively shorten the delivery process,not only improve the natural delivery rate of parturients,but also reduce the amount of postpartum hemorrhage,reduce the incidence of complications,and improve the outcome of delivery.
