摘要
目的探讨二尖瓣复合修复技术在风湿性二尖瓣病变患者中的应用,以期提高风湿性二尖瓣病变的修复成功率。方法选取了郑州市第七人民医院53例风湿性二尖瓣病变的患者,其中37例二尖瓣修复成功,修复过程分别采用了交界切开、增厚瓣叶的削薄、瓣下乳头肌的松解、次级腱索的切除、瓣叶的折叠、人工腱索的应用、二尖瓣成形环的应用等修复技术手段。结果对于本组病例本研究风湿性二尖瓣病变的修复成功率达到了69.8%,术后30天内1例病例死亡。随访时间12~36个月,平均(20±3)个月,随访率为85.7%,随访期间评估二尖瓣功能状态良好,没有因二尖瓣再狭窄及返流需再次手术的情况出现。结论风湿性二尖瓣的修复技术以处理粘连的交界,增厚的瓣叶及瓣下结构为主,同时根据病变情况复合应用其它技术,可获得很好的修复效果。
Objective To explore the application of mitral valve compound repair technique in patients with rheumaMethods Totally 53 cases of rheumatic mitral valve disease in our department were selected, of which 37 cases of mitral valve repair was successful, the repair process include commissurotomy, leaflet thinning, papillary muscle splitting, secondary chordal resection, leaflet plication, chordal replacement, annuloplasty ring and other repair techniques. Results For this group of cases, the repair success rate reached 69.8%, and 1 case died within 30 days after surgery. The follow-up period from 12 months to 36 months, an average of 20±3 months, follow-up rate of 85.7%, follow-up period to assess the function of the mitral valve is good, there is no patient need to be reoperated for valve re-stenosis or reflux. Conclusion The repair technique of rheumatic mitral valve is mainly to deal with the commissurotomy, leaflet thinning, papillary muscle splitting, and combine with other surgical procedures, and the postoperative outcomes were satisfactory.
