摘要
目的探讨三维斑点追踪技术(3D-STI)和二维斑点追踪技术(2D-STI)应用,评价不同部位心肌梗死患者左心室心肌应变。方法对我院收治的103例心肌梗死患者均行3D-STI和2D-STI检查,比较两种测得的不同部位心梗左室心肌纵向、径向、圆周应变均值,对比测出的左室舒张(EDV)、收缩末期容积(ESV)和射血分数(LVEF),左室心肌应变和功能以及图像进行分析。结果下壁心梗患者3D-STI检查与2D-STI相比,前者的LS更低、RS更高,差异有统计学意义(P<0.05),而两种方法所测前壁心梗患者各方向的应变均值无明显差异(P>0.05)。3D-STI技术对前壁心梗患者所测的EDV比2D-STI低,差异显著(P<0.05),两种方法测出的下壁心梗患者EDV、ESV和LVEF均无明显差别(P>0.05),3D-STI采集和分析图像的平均时间显著低于2D-STI(P<0.05)。结论3D-STI较2D-STI评价不同部位心肌梗死患者左心室的应变更为高效。
Objective To compare of three-dimensional speckle tracing technique and two-dimensional speckle tracing technique in evaluating left ventricular myocardial strain in patients with different myocardial infarction.Methods From April 2015 to April 2018,103 cases of myocardial infarction were analyzed in our hospital.All patients were examined by 3D-STI(3D-spot tracking technique)and 2D-STI(2D-spot tracking technique).The mean values of longitudinal,radial and circumferential strain of left ventricular myocardium in patients with myocardial infarction in different parts of myocardial infarction were compared.Results Compared with 2D-STI,the patients with inferior myocardial infarction had lower LS and higher RS,and the difference was statistically significant(P<0.05).However,there was no significant difference in strain mean in all directions between the two methods(P>0.05).The EDV measured by 3D-STI technique in patients with anterior wall myocardial infarction was lower than that of 2D-STI(P<0.05).There was no significant difference in EDV,ESV and LVEF between the two methods(P>0.05).The average time of 3D-STI acquisition and analysis was significantly lower than that of 2D-STI(P<0.05).Conclusion 3D-STI is more efficient than 2D-STI in evaluating the strain of left ventricle in patients with myocardial infarction.
