摘要
目的探讨三维时间飞跃法磁共振血管成像(3D-TOF MRA)技术在烟雾病(MMD)术前诊断及术后随访中应用价值。方法选取2016年09月至2019年09月松原吉林油田医院的21例MMD患儿。以DSA为检验标准,分析3D-TOF MRA对MMD患儿术前血管的诊断情况,术后重建血管成像质量、通畅性进行影像评分。结果3D-TOF MRA检出双侧颈内动脉(ICA)终末端21例,大脑前动脉(ACA)起始段21例,大脑中动脉(MCA)起始段20例,Willis环21例,烟雾血管18例,侧支血管14支。术后重建血管成像质量评分,3D-TOF MRA与MRI比较,差异无统计学意义;术后1个月随访,3D-TOF MRA显示重建血管通畅性评分(2.65±1.08)分。结论3D-TOF MRA对大血管及烟雾血管的检出情况较好,对术前诊断MMD及术后随访具有很高的应用价值。但对MMD早期烟雾血管的显示,仍需DSA确诊。
Objective To explore the application value of 3D-time of flight magentic resonance angiography(3D-TOF MRA)in the preoperative diagnosis and postoperative follow-up of moyamoya disease(MMD).Method From September 2016 to September 2019,21 children with MMD were selected from our hospital.Digital subtraction angiography(DSA)was used as the test standard to analyze the 3D-TOF MRA in the diagnosis of blood vessels in MMD children before operation,and the imaging quality and patency of reconstructed blood vessels after operation were evaluated.Result In 3D-TOF MRA,21 cases were detected in the end of internal carotid artery(ICA),21 in the beginning of anterior cerebral artery(ACA),20 in the beginning of middle cerebral artery(MCA),21 in Willis ring,18 in smog vessels and 14 in collateral vessels.There was no significant difference between 3D-TOF MRA and MRI in Journal of Vascular and Endovascular Surgery Vol.6,No.2,Mar 2020133 the quality score of reconstruction vascular imaging after operation.After 1 month follow-up,3D-TOF MRA showed the score of reconstruction vascular patency(2.65±1.08).Conclusion The detection of 3D-TOF MRA on large vessels and smoke vessels is similar to that of DSA,which has high application value for preoperative diagnosis of MMD and postoperative follow-up.However,DSA is still needed to diagnose the early smoke vessel display of MMD.
