摘要
目的探讨选择性胫神经腓肠肌内、外侧头肌支离断在小腿塑形中的应用。方法将2018年1月-2018年12月在我院整形外科治疗的50例小腿塑形患者随机分为两组,对照组使用A型肉毒毒素注射治疗,观察组使用选择性胫神经腓肠肌内、外侧头肌支离断术治疗,对比两组的术前术后小腿最大周径、小腿美观度及患者满意度、并发症发生率。结果观察组术后3个月、6个月、12个月的小腿最大周径明显小于对照组,缩小比例高于对照组(P<0.05);观察组术后12个月的小腿美观度评分、患者满意度评分均明显高于对照组(P<0.05);观察组感觉及运动障碍、瘢痕增生、色素沉着、腘动静脉损伤等并发症发生率高于对照组(P<0.05)。结论选择性胫神经腓肠肌内、外侧头肌支离断在小腿塑形中的应用效果显著,能有效改善小腿形态,获得良好塑形效果,且长时间不会反弹,但有一定并发症发生率,临床应辨清解剖结构,注意保护各神经分支,以免发生血管或神经误伤。
Objective To investigate the application of selective internal and lateral rupture of the tibiofibular nerve gastrocnemius in shaping the calf.Methods From January 2018 to December 2018,50 patients with calf plastic surgery in our hospital were randomly divided into two groups.The control group was treated with A botulinum toxin injection,the observation group was treated with selective tibial nerve gastrocnemius internal and lateral head muscle amputation.Compared the two groups with maximum leg circumference,the patients'satisfaction and complication rate,incidence of complications.Results After 3 months,6 months and 12 months,the maximum circumference of the lower leg in the observation group was significantly smaller than that in the control group (P <0.05).After 12months of operation, the calf aesthetic score and patient satisfaction score of the observation group were significantly higher thanthose of the control group (P <0.05).Complications such as sensory and motor disorders, scar hyperplasia, pigmentation and poplitealarteriovenous injury in the observation group were higher than those in the control group (P <0.05). Conclusion Selective tibial nervegastrocnemius internal and lateral head muscle branch break in the calf shape application is remarkable, can effectively improve theshape of the lower leg, obtain good shape effect, and long time will not rebound, but there is a certain incidence of complications, weshould distinguish the anatomical structure and protect the branches of each nerve so as to avoid blood vessel or nerve injury.
