摘要
目的探讨应用组织工程产品双层人工真皮修复指端及甲床缺损的临床效果。方法自2017年5月至2019年5月,我们对33例46指指端及甲床缺损伴有末节骨外露的病例,清创后应用双层人工真皮一期进行覆盖修复,未行自体皮移植。术后2~3周,指端及甲床覆盖的双层人工真皮血管化形成鲜红色的类真皮样组织及类甲床样组织。结果全部33例46指覆盖创面的人工真皮血管化良好。指端覆盖的人工真皮血管化后,肉芽组织生长良好,并自然上皮化,愈合良好,外形圆润饱满。甲床缺损覆盖的人工真皮血管化后,形成类甲床样组织,与原甲床衔接平整,指甲生长良好,较平整。结论应用组织工程产品双层人工真皮修复指端及甲床缺损与传统修复方法相比较,手术操作更加简单,创伤小,临床效果良好,值得推广应用。
Objective To investigate the clinical efficacy of bilayer artificial dermis of tissue engineering products for repair of fingertips and nail bed defects.Methods From May 2017 to May 2019, 33 cases of 46 fingertips and nail bed defects with distal phalanx exposure were treated. After debridement, bilayer artificial dermis was used for first stage coverage and repair, but no autograft was used. Two to three weeks after the operation, the bilayer artificial dermis covering the fingertips and nail beds was vascularized to form bright red dermoid like tissue and nail bed like tissue.Results All the 33 cases (46 fingers) had good vascularization. After the vascularization of the artificial dermis covering the fingertips, the granulation tissue grew well and was naturally epithelialized, with good healing and round and full appearance. After the vascularization of the artificial dermis covering the nail bed defects, the nail bed like tissue was formed, which was smoothly connected with the original nail bed, and the nail grew well and smooth.Conclusion Compared with the traditional methods, the bilayer artificial dermis of tissue engineering products for repair of fingertips and nail bed defects has the advantages of more simple operation, less trauma and better clinical efficacy, which is worth popularizing and applying.
