早期前列腺外周带癌与炎症的MRI鉴别诊断分析

摘要 目的分析早期前列腺外周带癌与炎症的MRI鉴别诊断。方法择取2016年1月至2018年12月,入本院就诊的早期前列腺外周带癌(癌症组)、前列腺炎(炎症组)患者各选择30例为研究对象,均经MRI检查,且经由病理学明确诊断,分析检查结果。结果癌症组T2WI中呈局灶结节样图像,且均匀低信号者18例(60.00%),炎症组T2WI中呈前列腺外周带见不均匀低信号,且为弥漫性分布者22例(73.33%);癌症组DCE-MRI曲线Ⅰ型、Ⅱ型、Ⅲ型分别为1例(3.33%)、10例(33.33%)、19例(63.33%);炎症组DCE-MRI曲线Ⅰ型、Ⅱ型、Ⅲ型分别为4例(13.33%)、17例(56.67%)、9例(30.00%);二者曲线类型分布数据存在显著性差异(χ2=7.1862,P=0.0275);癌症组Tmax、SImax分别为(18.10±2.35)s、(1.66±0.27)%、DWI ADC(0.94±0.14)×10-3mm2/s与炎症组Tmax、SImax的(20.95±2.43)s、(1.50±0.13)%间存在显著性差异(t=4.6178、2.9244,P=0.0000、0.0049)。结论MRI能够反映早期前列腺外周带癌与前列腺炎症病灶部位、结构等信息,具有良好的鉴别诊断价值,而通过DWI ADC值能够判断前列腺病变的良恶性。 Objective To analyze the differential diagnosis of early prostate cancer and inflammation by MRI.Methods From January 2016 to December 2018,30 patients with early prostate cancer(cancer group)and prostatitis(inflammation group)who were admitted to our hospital were selected for MRI.The diagnosis is confirmed by pathology,and the result of the examination is analyzed.Results In the cancer group,there were focal nodule images in T2WI,and 18 patients(60.00%)with uniform low signal.In the inflammatory group,T2WI showed uneven low signal in the peripheral zone of the prostate,and 22 patients with diffuse distribution(73.33%);cancer group DCE-MRI curve typeⅠ,typeⅡ,typeⅢtype were 1 case(3.33%),10 cases(33.33%),19 cases(63.33%);inflammation group DCE-MRI curve typeⅠ,typeⅡ,thpeⅢ.There were 4 cases(13.33%),17 cases(56.67%),and 9 cases(30.00%),respectively.There were significant differences in the curve type distribution data(χ2=7.1862,P=0.0275);cancer group Tmax,SImax They were(18.10±2.35)s,(1.66±0.27)%,DWI ADC(0.94±0.14)×10-3 mm2/s,and between Tmax and SImax(20.95±2.43)s and(1.50±0.13)%of the inflammation group.There were significant differences(t=4.6178,2.9244,P=0.0000,0.0049).Conclusion MRI can reflect the location and structure of early prostate cancer and prostatic inflammatory lesions,and has good differential diagnosis value.The DWI ADC value can be used to judge the benign and malignant prostate lesions.
