摘要
目的:探讨β2-微球蛋白(β2M)、糖化血红蛋白(HbA1c)、脂蛋白相关磷脂酶A2(Lp-PLA2)、磷酸葡萄糖变位酶(PGM)联合检测诊断冠心病(CHD)的价值。方法:选择135例CHD患者为CHD组,另选择本院同期健康体检者70例作为对照组,收集血液样本,放射免疫法检测β2M,免疫比浊法检测HbA1c,酶联免疫吸附法检测Lp-PLA2、PGM,采用Logistics回归分析CHD发生的相关因素,采用ROC分析诊断试验的灵敏度和特异度。结果:CHD组血清β2M、HbA1c、Lp-PLA2、PGM水平均显著高于对照组(P<0.05);Logistics回归分析显示,高水平β2M、HbA1c、Lp-PLA2、PGM是发生CHD的危险因素;β2M(临界值2.65μg·mL^-1)、HbA1c(临界值5.00%)、Lp-PLA2(临界值197.20 ng·mL^-1)、PGM(临界值34.52 U·L^-1)联合检测诊断CHD的AUC为0.949,准确度为95.12%,灵敏度为95.56%,特异度为94.29%,均明显高于单项检测。结论:CHD患者β2M、HbA1c、Lp-PLA2、PGM水平显著升高,联合检测诊断CHD的准确度和灵敏度更高。
Objective:To explore the accuracy and sensitivity of combined detection ofβ2-microglobulin(β2M),glycosylated hemoglobin(HbA1c),lipoprotein-associated phospholipase A 2(Lp-PLA2)and phosphoglucomutase(PGM)in the diagnosis of coronary heart disease(CHD).Methods:135 CHD patients admitted as CHD group while 70 healthy examiners in the hospital in the same period were selected as the control group.The blood samples were collected,andβ2M was detected by radioimmunoassay,and HbA1c was detected by immunoturbidimetry,and Lp-PLA2 and PGM were detected by enzyme-linked immunosorbent assay,and logistics regression analysis was performed on the related factors affecting the occurrence of CHD.The sensitivity and specificity of diagnostic tests were analyzed using ROC analysis.Results:The serum levels ofβ2M,HbA1c,Lp-PLA2 and PGM in CHD group were significantly higher than those in control group(P<0.05).Logistics regression analysis showed that high levels ofβ2M,HbA1c,Lp-PLA2 and PGM were risk factors for CHD occurrence.The AUC,accuracy,sensitivity and specificity of combined detection of critical valueβ2M(critical value of 2.65μg·mL^-1),HbA1c(critical value of 5.00%),Lp-PLA2(critical value of 197.20 ng·mL^-1)and PGM(critical value of 34.52 U·L^-1)were 0.949,95.12%,95.56%and 94.29%in the diagnosis of CHD,which were significantly higher than those of single detection.Conclusion:The levels ofβ2M,HbA1c,Lp-PLA2 and PGM in CHD patients are significantly increased.Combined detection has higher accuracy and sensitivity in the diagnosis of CHD.
