摘要
目的:从中医角度分析新型冠状病毒肺炎患者的临床特征。方法:纳入2020年1月5日—2月8日收治的新型冠状病毒肺炎确诊患者50例,采集并分析入选研究对象的流行病学资料、中医证候信息、胸部CT报告、人口学资料、血液检查数据等。结果:入选的50例研究对象中,24%患者出现白细胞总数降低,26%患者出现淋巴细胞百分比降低,60%患者出现C-反应蛋白增高,34%患者出现红细胞沉降率增高,16%患者出现转氨酶增高,胸部CT显示,92%患者呈现多发小斑片影,或多发磨玻璃影、浸润影,少数患者呈现进展性,符合病毒性肺炎胸部。舌质为淡红及红色比例分别为48%及38%,暗红舌为14%;证型中湿毒郁肺型占84%,热毒闭肺型占16%;滑脉所占比例高于数脉;腻苔及白苔分别占46%及54%。湿毒郁肺型患者平均年龄低于热毒闭肺型患者(P<0.05);湿毒郁肺型患者男性患者人数多于热毒闭肺型患者(P<0.05)。结论:2019-nCoV肺炎患者多以湿毒郁肺型为主,病理因素与湿、毒、热、虚相关,湿毒是其病理核心。
Objective:To analyze the clinical feature of COVID-19 from the perspective of TCM.Methods:50 patients diagnosed with COVID-19 were included in the study.The epidemiological data,TCM syndrome information,chest CT report,demographic data and blood test data were collected for analysis.Results:Among the 50 selected subjects,24%of the patients had a decrease in the total number of white blood cells,26%of the patients had a decrease in the percentage of lymphocytes,60%of the patients had an increase in C-reactive protein,34%of the patients had an increase in red blood cell sedimentation rate,and 16%of the patients had an increase in transaminase.Chest CT showed that 92%of the patients presented multiple small patchy shadows,or multiple ground glass shadows and infiltrates.A few patients showed progressive deterioration,which was consistent with viral pneumonia chest.Light-red tongue and red tongue accounted for 48%and 38%respectively,and dark-red tongue accounted for 14%.The syndrome of dampness-toxin depressing Lung accounted for 84%,and the type of heat-toxin blocking Lung accounted for 16%.The proportion of slippery pulse is higher than the number of fast pulse.46%were greasy coating and 54%were white coating.The average age of the patients with damp-toxin stagnation was lower than that of the patients with heat-toxin stagnation(P<0.05).The dampness-toxin stagnation pattern was more than heat-toxin stagnation pattern in male patients(P<0.05).Conclusion:The most pattern of COVID-19 is dampness-toxin depressing Lung.The pathological factors are related to dampness,toxin,heat and deficiency,among which damp-toxin is the core pathogenesis.
