患者感知医患共情对慢阻肺患者炎症因子的影响:焦虑、自我效能、睡眠的中介作用 被引量:26

Patients’Perception of Doctor-Patient Empathy and Inflammatory Factors in COPD Patients:the Mediating Effects of Anxiety,Self-efficacy and Sleep

摘要 目的:探讨医患共情对住院慢阻肺患者的焦虑、自我效能、睡眠、IL-6水平的影响及其作用机制。方法:本研究包括215名慢阻肺患者。出院三个月后,采用诊疗关系共情量表(CARE)、医院焦虑抑郁量表(HADS)、一般自我效能量表(GSES)和多导睡眠图(PSG)测量患者感知医患共情以及患者的焦虑、自我效能和睡眠质量。运用ELISA法测定血清IL-6水平。结果:出院三个月后,患者感知医患共情与焦虑(r=-0.462,P<0.01)和IL-6(r=-0.397,P<0.01)呈负相关,与自我效能(r=0.436,P<0.01)和睡眠质量(r=0.434,P<0.01)呈正相关。患者焦虑、自我效能和睡眠质量是共情对IL-6影响的中介变量。结论:焦虑、自我效能、睡眠、患者感知医患共情、IL-6水平密切相关,焦虑、自我效能、睡眠在患者感知医患共情对IL-6的影响中起中介作用。 Objective:To explore the effect of patients’perception of empathy between doctors and patients on anxiety,self-efficacy,sleep and IL-6 level in hospitalized COPD patients and its mechanism.Methods:This study included 215 COPD patients.Three months after discharge,the consultation and relational empathy(CARE)scale,Hospital Anxiety and Depression Scale(HADS),General Self-Efficacy Scale(GSES)and Polysomnography(PSG)were used to measure patients’perception of empathy between doctors and patients,anxiety,self-efficacy and sleep quality of patients.The IL-6 level in serum was determined by ELISA.Results:Three months after discharge,patients’perception of empathy between doctors and patients was negatively correlated with anxiety(r=-0.462,P<0.01)and IL-6(r=-0.397,P<0.01),and positively correlated with self-efficacy(r=0.436,P<0.01)and sleep quality(r=0.434,P<0.01).Anxiety,self-efficacy and sleep quality were mediators of empathy on IL-6.Conclusion:Anxiety,self-efficacy,sleep,empathy between doctors and patients and IL-6 are closely related.Anxiety,self-efficacy and sleep play a mediating role in the influence of empathy between doctors and patients on IL-6 in COPD patients.
