颅脑CT平扫诊断颅内静脉窦血栓 被引量:3

摘要 目的评价颅脑CT平扫诊断急性颅内静脉窦血栓的准确性。方法回顾性分析35例临床可疑颅内静脉窦栓塞患者的影像学资料,以颅脑CTV为诊断标准,评价颅脑CT平扫诊断颅内静脉窦血栓的准确性。测量静脉窦的CT值,并比较患者组与正常对照组之间是否具有差异。结果14例患者诊断为颅内静脉窦血栓,CT平扫诊断的敏感性和特异性为100%、79%。患者组的平均静脉窦CT值为66.5HU,正常对照组为53.4HU,两者之间差异有统计学意义。ROC分析显示当临界值为58.5HU时,诊断颅内静脉窦血栓的敏感性和特异性分别为93%、86%。结论颅脑CT平扫作为急性颅内静脉窦血栓首诊的检查方法,具有较高的诊断价值,结合静脉窦CT值的测量可以提高诊断的准确性。 Objective The aim of the study was to investigate the value of non-contrast head CT scan in the diagnosis of cerebral venous sinus thrombosis(CVST).Methods Retrospectively analysis the imaging of patients with clinical signs of CVST.CT venography was used as a reference standard.Attenuation inside the cerebral venous sinuses was measured and compared between the patient and the control group.Results CVST was confirmed in 14 patients.Sensitivity and specificity of CT Scan for overall presence of CVST were 100%and 79%,respectively.The attenuation values between CVST patients and control group were significantly different(66.5±7.7 HU vs 53.4±5.6 HU).The ROC analysis showed an area under the curve(AUC)of 0.93 and an optimal cutoff value of 58.5 HU,leading to a sensitivity of 93%and specificity of 86%.Conclusion Non-contrast head CT scan as a first-line investigation has a high value for diagnosis of CVST in the emergency setting.The measurement of the sinus attenuation may improve the diagnostic value of the examination.
