摘要
目的:观察干化学法联合尿沉渣联合用于尿常规检验的应用效果。方法:选取2016年4月~2018年9月某院接诊的行尿常规检验的尿液标本217份,分别采用干化学法、尿沉渣镜检与上述两种方式联合检测的方式对尿液标本进行检测,比较定量白细胞和定性红细胞阳性率,结合尿液标本培养结果,分析尿沉渣法、干化学检测与两种方式联合检测在尿常规检验中的准确性。结果:干化学检验定量白细胞阳性率(7.37%)明显低于尿沉渣法的定量白细胞阳性率(36.41%)(P<0.05),定量红细胞阳性率(15.67%)明显高于尿沉渣的定量红细胞阳性率(5.99%)(P<0.05);联合检测的定量白细胞阳性率(43.32%)和定量红细胞阳性率(20.74%)显著高于干化学检测结果和尿沉渣结果(P<0.05);不同检测方法在尿常规检验中的敏感性、特异性、准确性、漏诊率、误诊率依次如下,干化学检测:90.7%、85.3%、88.0%、9.3%、14.7%,尿沉渣:91.7%、89.0%、90.3%、8.3%、11.0%,上述两种方法联合检测:98.1%、96.3%、97.2%、1.9%、3.7%。结论:干化学法联合尿沉渣检验可有效降低尿常规检验误诊率和漏诊率,为医师临床诊疗患者病情提供更加准确的参考依据,具有重要的临床应用价值。
Objective:To observe the application effect of dry chemistry combined with urine sediment in routine urine test.Methods:217 urine samples for routine urine tests in a hospital from April 2016 to September 2018 were tested,dry chemistry method,urinary sediments examination and the above two methods combined were used to examine the urine samples,respectively.The positive rates of quantitative white blood cells and qualitative red blood cells were compared.The positive rates of quantitative leukocyte and qualitative erythrocyte were compared,and the accuracy of urine sediment method,dry chemical test and two methods in routine urine test was analyzed based on the results of urine culture.Results:The positive rate of quantitative leucocyte(7.37%)was significantly lower than that of quantitative leucocyte(36.41%)by dry chemical test(P<0.05),and the positive rate of quantitative red blood cell(15.67%)was significantly higher than that of quantitative red blood cell(5.99%)by urinary sediment method(P<0.05).The positive rate of quantitative white blood cells(43.32%)and quantitative red blood cells(20.74%)detected jointly were significantly higher than those detected by dry chemistry and urine sediment(P<0.05).The sensitivity,specificity,accuracy,missed diagnosis rate and misdiagnosis rate of different detection methods in routine urine test are as follows:dry chemical detection:90.7%,85.3%,88.0%,9.3%,14.7%.Urine sediment:91.7%,89.0%,90.3%,8.3%,11.0%.The above two methods are combined for detection:98.1%,96.3%,97.2%,1.9%,3.7%.Conclusion:Dry chemical method combined with urine sediment test can effectively reduce the misdiagnosis rate and missed diagnosis rate of routine urine test,provide more accurate reference basis for doctors to diagnose and treat patients'illness,and has important clinical application value.
