摘要
目的探讨前列腺癌(Pca)患者全身骨显像与血清T、D-D、IL-6和PSA水平相关性的临床价值。方法对126例Pca患者进行99mTcO4--MDP全身骨显像,与115例良性前列腺患者测定血清T、D-D、IL-6和PSA水平并进行对比性分析。Logistic回归模型分析独立的相关因素。ROC曲线预估了局限性Pca向Pca合并骨转移患者转化的临床意义。结果 126例Pca患者全身骨显像灵敏度为97. 59%、特异度为98. 11%和准确率为98. 45%。65例Pca合并骨转移患者较之61例局限性Pca患者血清D-D、IL-6和PSA水平明显增高,血清T水平降低。Logistic回归模型分析表明:血清T、D-D、IL-6和PSA水平增高与Pca患者全身骨显像密切相关。血清IL-6水平为最佳,其次为PSA、D-D和T,均具有较好的预测临床价值。结论99mTcO4--MDP全身骨显像是诊断Pca患者骨转移的方法,并与血清T、D-D、IL-6和PSA水平联检是早期诊断骨转移的有价值指标,具有评估局限性Pca向Pca合并骨转移的临床意义。
Objective To explore the clinical value of the relationship between99mTcO4--MDP whole body bone imaging and serum testosterone(T),D-dimers(D-D),interleukin-6(IL-6)and prostate specific antigen(PSA)levels in patients with prostate cancer(Pca).Methods Wm TcO4-MDP whole body bone imaging was done in 126 patients with Pca.The serum T,D-D,IL-6 and PSA levels were determined in 115 patients with benign prostate and compared for analysis.Logistic regression model was used to analyze the unique relation factor.The clinical significance of localized Pca transferring to Pca with bone metastases was predicted by ROC curve.Results The sensitivity,specitivity and precision of whole body bone imaging in 126 patients with Pca were respectively 97.59%,98.11%,98.45%.The serum D-D,IL-6 and PSA levels in 65 patients complicated with localized Pca were significantly higher than that in 61 patients with localized Pca,but serum T level decreased.Logistic regression model analysis showed closely relation between whole body bone imaging and serum T,D-D,IL-6 and PSA.Serum IL-6 level was the best,followed by PSA,D-D and T,all of which had good predictive clinical value.Conclusion 99mTcO4--MDP whole body bone imaging is a method to diagnose osseous metastasis in patients with Pca,and the combined detection of T,D-D,IL-6 and PSA are a valuable index in the early diagnosis of bone metastases and has clinical significance in predicting localized Pca transferring to Pca with bone metastases.
