角膜塑形镜配戴初期使用重组牛碱性成纤维细胞生长因子眼用凝胶的疗效 被引量:7

Effect of Early Use of Recombinant Bovine Basic Fibroblast Growth Factor(Befoushu) Eye Gel in the Early Stage of Corneal Plastic Lens Wear

摘要 目的研究探讨角膜塑形镜配戴初期使用重组牛碱性成纤维细胞生长因子眼用凝胶的疗效。方法选择某院眼科2016年6月3日-2018年6月28日就诊的青少年近视患者130例(260只眼),随机分为2组各65例(130只眼),对照组给予角膜塑形镜配戴,观察组在角膜塑形镜配戴初期给予重组牛碱性成纤维细胞生长因子眼用凝胶滴眼,比较两组的裸眼视力、眼屈光度数、眼压、眼轴长度、泪液分泌量、泪膜破裂时间、角膜荧光染色评分、并发症发生率、生活质量评分。结果 (1)治疗6个月后,观察组的裸眼视力(0.28±0.10)、泪液分泌量(10.75±2.89)、生活质量各维度评分(87.47±8.29、86.89±8.21、87.16±8.24、85.46±8.45)均较对照组更高(t=5.393、4.853、5.320、5.988、5.705、5.271,P<0.05),其眼屈光度数(-4.10±0.32)、角膜荧光染色评分(4.09±1.23)均较对照组更低(t=5.950、6.042,P<0.05),其泪膜破裂时间(10.81±4.47)较对照组更长(t=6.508,P<0.05)。(2)对照组、观察组治疗前后的眼压、眼轴长度比较,差异均无统计学意义(t=0.400、0.497、0.142、0.271,P>0.05)。(3)治疗6个月内,观察组的角膜上皮点染、角膜浸润、夜间眩光等发生率分别为3.08%、1.54%、4.62%,均较对照组更低(χ~2=4.866、4.795、4.188,P<0.05)。结论角膜塑形镜配戴初期应用重组牛碱性成纤维细胞生长因子眼用凝胶,可有效矫正近视患者的视力和眼屈光度数,改善其泪液分泌和角膜染色情况,有利于减少角膜并发症的发生。 Objective To study the effect of using recombinant bovine basic fibroblast growth factor(BbFGF)eye gel in the early stage of corneal plastic lens wear.Methods From June 3,2016 to June 28,2018,130 juvenile myopia patients(260 eyes) were selected in a Hospital,which were randomly divided into two groups:65 cases each(130 eyes),the control group was given corneal plastic lens(130 eyes),and the observation group was given gel-drop eye to bed in the early stage of corneal plastic surgery.Results(1) After 6 months of treatment,the naked visual acuity and tear secretion were(0.28±0.10) and(10.75±2.89),respectively.QOL scores(87.47±8.29,86.89±8.21,87.16±8.24,85.46±8.45) were higher than those in the control group(t=5.393,4.853,5.320,5.988,5.705,5.271,P<0.05).The ocular refractive index(-4.10±0.32) and the corneal fluorescent staining score(4.09±1.23) were lower than those in the control group(t=5.950,6.042,P<0.05),and the tear film rupture time(10.81±4.47) was longer than that in the control group(t=6.508,P<0.05).(2) In the control group,there was no significant difference in IOP and axial length between before and after treatment(t=0.400,0.497,0.142,0.271,P>0.05).(3) Within 6 months after treatment,the incidence of corneal epithelial spot staining,corneal infiltration and nocturnal glare were 3.08%,1.54% and 4.62% respectively in the observation group,which were lower than those in the control group(χ~2=4.866,4.795,4.188,P<0.05).Conclusion The application of befushu eye gel in the early stage of corneal plastic lens can effectively correct the visual acuity and diopter of myopia,improve the tear secretion and corneal staining,and reduce the occurrence of corneal complications.
