人工真皮联合自体刃厚皮移植在特重度烧伤后期功能重建中的应用研究 被引量:5

Clinical application of artificial dermis combined with autologous blade thickness skin grafting in functional reconstruction of severe burns

摘要 目的研究自体刃厚皮移植联合人工真皮应用于特重度烧伤后期功能重建的效果。方法选取2016年9月至2018年8月在周口港区医院治疗的60例特重度烧伤患者,将其分为两组,每组30例。对照组采取Ⅰ期自体薄中厚皮移植方案治疗,观察组采取Ⅰ期人工真皮植入、Ⅱ期自体刃厚皮移植方案治疗。比较两组患者的自体皮成活率、感染坏死情况、受皮区及供皮区完全愈合时间以及VSS评分等指标。结果两组患者术后5 d自体皮成活率、皮片感染坏死情况、受皮区愈合时间等比较,差异均未见统计学意义(P>0.05);与对照组对比,观察组患者术受皮区、供皮区温哥华瘢痕量表(VSS)评分更低,且供皮区愈合时间更短(P<0.05)。结论自体刃厚皮移植与人工真皮联合治疗有利于供皮区愈合,减少受皮区及供皮区瘢痕形成,具有明显优势。 Objective To study the effect of autogenous blade thick skin transplantation combined with artificial dermis on functional reconstruction in the late stage of severe burn.Methods From September 2016 to August 2018,60 patients with severe burn were selected.They were divided into two groups,with 30 cases in each group.The control group was treated with one stage of thin and medium thick skin autograft.In the observation group,the first stage of artificial dermis implantation and the second stage of self blade thick skin transplantation were used.The survival rate of autogenous skin,infection and necrosis,the time of complete healing of recipient and donor skin,and the score of VSS were compared between the two groups.Results There was no significant difference between the two groups in the survival rate of autogenous skin,the infection and necrosis of skin,the healing time of recipient area(P>0.05);compared with the control group,the Vancouver Scar Scale(VSS)score of recipient area and donor area in the observation group was lower,and the healing time of donor area was shorter(P<0.05).Conclusions The combination of autogenous blade thick skin transplantation and artificial dermis is beneficial to the healing of donor skin and reduce the formation of scar in recipient skin and donor skin.
