摘要
目的调查天津铁路健康体检员工的血清胃蛋白酶原(PG)水平,探讨其变化规律。方法选取2019年3月~11月于天津铁路疾病预防控制中心进行健康体检的铁路员工49523例,采用酶联免疫吸附法检测其血清胃蛋白酶原Ⅰ(PGⅠ),胃蛋白酶原Ⅱ(PGⅡ)的水平并计算PGⅠ/PGⅡ比值(PGR),应用spss19.0统计软件分析比较不同性别、不同年龄(<31岁,31~40岁,41~50岁和>50岁),不同工种间铁路员工PG的差异。结果铁路员工血清PGⅠ,PGⅡ和PGR水平M(P25,P75)分别为81.72(67.31,97.08)ng/ml,7.6(4.98,11.82)ng/ml和10.55(7.05,15.52)。男性组PGⅠ,PGⅡ和PGR水平M(P25,P75)分别为82.66(68.24,97.95)ng/ml,7.82(5.19,12.01)ng/ml和10.36(6.97,15.14),女性组分别为74.19(60.94,88.96)ng/ml,5.64(3.70,9.82)ng/ml和12.71(7.96,18.45),差异均有统计学意义(U=90670471.00~139064629.00,P=0.000)。不同年龄组间血清PG的水平差异均有统计学意义(H=1269.02~3779.45,P=0.000)。随着年龄的增高,血清PGⅠ,PGⅡ水平升高和PGR降低。比较不同工种间血清PG的水平差异,一线员工PGⅠ水平要高于附属员工,差异有统计学意义(U=65095021.50,P=0.000);PGⅡ和PGR水平的差异无统计学意义(U=68566462.50,67380108.50,P=0.731,0.055)。相关性分析结果显示,PGⅠ和PGⅡ水平与年龄呈正相关(rs=0.161,0.284,P<0.05);PGR与年龄呈负相关(rs=-0.234,P<0.05)。结论天津铁路疾病预防控制中心健康体检员工血清PGⅠ,PGⅡ和PGR水平呈明显的偏态分布,在不同性别、不同年龄组间其差异有统计学意义,而不同工种间的差异需进一步研究。
Objective To investigate the serum pepsinogen(PG)levels in Tianjin Railway staff for physical examination and study the variable rules.Methods 49523 railway staff were selected from March to November 2019 for physical examination in Center for Disease Control and Prevention of Tianjin Railway.The levels of serum pepsinogenⅠ(PGⅠ)and pepsinogenⅡ(PGⅡ)were detected by enzyme-linked immunosorbent assay(ELISA),and the ratio of PGⅠto PGⅡ(PGR)was calculated.The differences were compared between different genders,among different age groups(<31,31~40,41~50 and>50)and different types of worker groups by SPSS19.0 statistical software.Results The M(P25,P75)of serum PGⅠ,PGⅡand PGR among railway staff were 81.72(67.31,97.08)ng/ml,7.6(4.98,11.82)ng/ml and 10.55(7.05,15.52)respectively.The levels of PGⅠ,PGⅡand PGR in male group were 82.66(68.24,97.95)ng/ml,7.82(5.19,12.01)ng/ml and 10.36(6.97,15.14)respectively,which in female group were 74.19(60.94,88.96)ng/ml,5.64(3.70,9.82)ng/ml and 12.71(7.96,18.45).There were significant differences between two gender groups(U=90670471.00~139064629.00,P=0.000)and among different age groups(H=1269.02~3779.45,P=0.000).With the ages increasing,the levels of serum PGⅠand PGⅡincreased,while PGR decreased.The level of serum PGⅠin the front-line workers was higher than that in the affiliated workers(U=65095021.50,P=0.000),while there were no significant differences in the levels of PGⅡand PGR(U=68566462.50,67380108.50,P=0.731,0.055).The results of correlation analysis showed that the PGⅠand PGⅡwere positively correlated with age(rs=0.161,0.284,P<0.05),and PGR was negatively correlated with age(rs=-0.234,P<0.05).Conclusion The levels of serum PGⅠ,PGⅡand PGR in Railway staff for physical examination of Tianjin Railway Centers for Disease Control and Prevention showed obviously skewed distribution with significant differences between genders,among age groups.The differences between two types of work groups need further research.
