膝关节强化控制训练对脑卒中后偏瘫患者下肢运动功能、平衡及步行能力的改善作用 被引量:7

摘要 目的 探讨膝关节强化控制训练对脑卒中后偏瘫患者下肢运动功能、平衡及步行能力的改善作用.方法 选取2017年1月~2018年12月康复科就诊脑卒中后偏瘫患者86例,随机分为干预组和对照组各43例.两组患者均予控制颅内压、血压、血糖和血脂,营养脑细胞及预防并发症等基础治疗.对照组患者予常规康复训练方法训练,干预组患者在对照组基础上加用膝关节强化控制训练,两组患者均干预8周.观察并比较两组患者干预前与干预8周后下肢运动功能、平衡能力及步行能力的变化.结果 干预8周后,两组患者FMA评分、BBS静态和动态评分评分较前明显上升(P<0.05或P<0.01),且干预组患者上升幅度较对照组更明显(P<0.05);两组患者FAC评分和MWS较前明显上升(P<0.05或P<0.01),且干预组患者上升幅度较对照组更明显(P<0.05).结论 膝关节强化控制训练用于脑卒中后偏瘫患者可促进下肢运动功能恢复,改善其平衡和步行能力. Objective To investigate the improvement effect of knee joint intensive control training on lower limb motor function,balance and walking ability in patients with hemiplegia after stroke.Methods 86 patients with hemiplegia after stroke in the rehabilitation department from January 2017 to December 2018 were randomly divided into intervention group and control group,with 43 cases in each group.Both groups of patients were treated with basic treatments such as controlling intracranial pressure,blood pressure,blood sugar and blood lipids,nutritional brain cells and prevention of complications.The patients in the control group were trained in routine rehabilitation training.The patients in the intervention group were given knee joint intensive control training on the basis of the treatment in the control group.Both groups were treated for 8 weeks.The changes of lower extremity motor function,balance ability and walking ability were observed and compared between the two groups before and after 8 weeks of intervention.Results After 8 weeks of intervention,the FMA score,BBS static and dynamic scores of the two groups were significantly higher than those before(P<0.05 or P<0.01),and the increase rate of the intervention group was more significant than that of the control group(P<0.05).The FAC score and MWS of the patients in two groups were significantly higher than those before(P<0.05 or P<0.01),and the increase rate of the intervention group was more significant than that of the control group(P<0.05).Conclusion Knee joint intensive control training for patients with hemiplegia after stroke can promote the recovery of lower extremity motor function and improve their balance and walking ability.
