111例患者奋乃静血药浓度监测分析

摘要 目的:了解临床使用奋乃静患者的血药浓度监测情况,为临床合理用药提供依据。方法:收集2018年6月至2020年2月首都医科大学附属北京安定医院(以下简称“我院”)服用奋乃静并进行血药浓度监测患者的临床资料,包括年龄、性别、服药剂量、联合用药及相关不良反应等。回顾性分析奋乃静血药浓度分布、性别、年龄、服药剂量及不良反应与患者血药浓度的关系。结果:111例患者共有监测数据229例次,仅监测1次血药浓度的患者有61例(占54.95%),通过血药浓度监测调整剂量并进行多次监测的患者有50例(占45.05%);奋乃静平均血药浓度为(2.42±2.73)ng/ml,但个体差异较大,37.84%的患者(42例)在有效血药浓度范围内。不同性别患者间奋乃静血药浓度的差异无统计学意义(P>0.05);奋乃静血药浓度呈现随患者年龄增加而先增加后减少的趋势,但差异无统计学意义(P>0.05);随着患者用药剂量的增加,奋乃静血药浓度升高,差异具有统计学意义(P<0.05)。结论:奋乃静血药浓度个体差异大,儿童、青少年及老年患者的血药浓度相对较低,应加强药物监测,根据测得的血药浓度制订个体化的给药方案,选择适宜的给药剂量,在保证疗效的同时减少不良反应的发生。 OBJECTIVE:To investigate the monitoring on blood concentration of perphenazine,so as to provide references for clinical rational drug application.METHODS:Clinical data of patients taking perphenazine and monitored blood concentration in Beijing Anding Hospital Affiliated to Capital Medical University(hereinafter referred to as“our hospital”)from Jun.2018 to Feb.2020 were collected,including age,gender,dose,combined medication and averse drug reactions.To retrospectively analyze the correlation between patients'blood concentration and the distribution of blood concentration of perphenazine,patients'gender,age,dose and adverse drug reactions.RESULTS:A total of 229 case-times of monitoring data were collected from 111 patients,61 patients(54.95%)were monitored the blood concentration for only once,50 patients(45.05%)had been adjusted dosage and repeated monitoring through blood concentration monitoring;the average blood concentration of perphenazine was(2.42±2.73)ng/ml,but the individual difference was large,37.84%of patients(42 cases)were within the effective blood concentration range.There was no statistical significance in difference in the concentration of perphenazine between patients of different genders(P>0.05);the blood concentration of perphenazine showed a trend of increasing first and then decreasing with the increasing of patients'ages,while the difference had no statistical significance(P>0.05);with the increase of medication dose,the blood concentration of perphenazine also increased,the difference was statistically significant(P<0.05).CONCLUSIONS:The blood concentration of perphenazine varies greatly among individuals,the blood concentrations of children,adolescents and elderly patients are relatively lower.It is necessary to strengthen drug monitoring and formulate an individualized dosage regimen based on the measured blood concentration,to select appropriate dose to reduce the incidence of adverse drug reactions while ensure efficacy.
