摘要
目的研究慢性中耳炎(CSOM)致病菌菌群分布特点以及主要感染菌株金黄色葡萄球菌耐药情况,为CSOM治疗提供指导。方法回顾性分析2016年1月-2018年12月本院237例CSOM细菌培养鉴定阳性结果,统计分析检出病原体及金黄色葡萄球菌的药敏试验。结果237例CSOM患者检出致病菌230株,其中革兰阳性菌182例(79.13%),以金黄色葡萄球菌(53.91%)为主;革兰阴性菌48例(20.87%),以铜绿假单胞菌(8.26%)为主。124株金黄色葡萄球菌对甲氧西林敏感菌株102例,耐甲氧西林金葡萄球菌22株,耐甲氧西林金葡萄球菌检出率为17.7%;金黄色葡萄球菌对青霉素、红霉素、克林霉素耐药较为严重,对替考拉宁、万古霉素、利福平较为敏感,3年左氧氟沙星、环丙沙星、苯唑西林耐药率均增高(P<0.05)。结论对CSOM患者感染菌群分布及耐药的动态变迁研究有利于临床的合理用药,在治疗疗效提高及减少耐药菌株方面均有重要的临床应用价值。
Objective To study the distribution characteristics of pathogenic bacteria in chronic otitis media(CSOM)and the drug resistance of Staphylococcus aureus,the main infectious strains,in order to provide guidance for the treatment of CSOM.Methods Totally,237 cases of CSOM in our hospital from January 2016 to December 2018 were analyzed retrospectively.The drug susceptibility tests of pathogens and Staphylococcus aureus were analyzed.Results A total of 230 strains of pathogenic bacteria were detected in 237 CSOM patients,of which 182(79.13%)were Gram-positive bacteria,mainly Staphylococcus aureus(53.91%)and 48(20.87%)were Gram-negative bacteria,mainly Pseudomonas aeruginosa(8.26%).124 strains of Staphylococcus aureus were sensitive to methicillin in 102 cases,22 strains of Staphylococcus aureus were resistant to methicillin,the detection rate of Staphylococcus aureus was 17.7%.Staphylococcus aureus was more resistant to penicillin,erythromycin and clindamycin,more sensitive to teicoplanin,vancomycin and rifampicin,and three years left.The drug resistance rates of ofloxacin,ciprofloxacin and oxacillin increased(P<0.05).Conclusion The study on the distribution of infectious flora and the dynamic change of drug resistance in CSOM patients is beneficial to rational drug use in clinic,and has important clinical application value in improving therapeutic efficacy and reducing drug-resistant strains.
