摘要
目的调查分析连续性血液净化(CBP)非计划撤机的风险因素及其护理管理对策。方法选取我院2017年1月至2019年5月147例CBP治疗患者为研究对象,采用我院自制的基本资料调查表统计发生非计划撤机76例,采用单因素和多因素logistic回归分析CBP非计划撤机的风险因素并给予相应护理管理对策。结果多因素logistic回归分析结果显示,年龄(OR=2.890)、体外循环抗凝(OR=2.377)、发生低体温(OR=2.482)、因意识障碍等出现躁动(OR=3.142)、脱水速度(OR=3.260)、护理人员体位管理(OR=2.795)是导致CBP非计划撤机的独立危险因素(P<0.05)。结论CBP治疗非计划撤机的危险因素较多且复杂,通过评估护患因素、设备因素或突发性问题因素,加强体温和体位护理,动态调整循环抗凝方式与脱水速度,可有效降低非计划撤机发生率。
Objective To investigate and analyze the unplanned hemodialysis machine weaning risk factors and its nursing management countermeasures in patients with continuous blood purification(CBP).Methods To select 147 cases of patients with CBP treatment in our hospital from January 2017 to May 2019 as the study objects.76 cases of unplanned hemodialysis machine weaning were counted by using the self-made basic data questionnaire of our hospital.Univariate and multivariate logistic regression analysis was used to analyze the risk factors of unplanned hemodialysis machine weaning in patients with CBP and give corresponding nursing management countermeasures.Results Multivariate logistic regression analysis showed that the age(OR=2.890),extracorporeal circulation anticoagulation(OR=2.377),hypothermia(OR=2.482),agitation due to disturbance of consciousness(OR=3.142),dehydration speed(OR=3.260),and posture management of nursing staff(OR=2.795)were the independent risk factors that led to the unplanned hemodialysis machine weaning in patients with CBP(P<0.05).Conclusion The risk factors of unplanned hemodialysis machine weaning of CBP treatment were many and complicated.By evaluating factors of nursing and the patients,equipment factors or sudden problem factors,strengthening nursing for body temperature and posture,dynamically adjusting circulating anticoagulation methods and dehydration speed,which can effectively reduce the incidence rate of unplanned hemodialysis machine weaning.
