交叉配血中抗人球蛋白微柱凝胶法、凝聚胺法的配血结果及差异比较研究 被引量:29

摘要 目的分析交叉配血中抗人球蛋白微柱凝胶法、凝聚胺法的配血结果及差异。方法选取本院2018年1月至2018年12月需输血治疗的486例患者为研究对象,分别通过抗人球蛋白微柱凝胶法、凝聚胺法行交叉配血,比较两种不同配血试验结果、差异,同时分析交叉配血不匹配的病因。结果微柱凝胶法配血时间显著长于凝聚胺法(P<0.05);微住凝胶法总阳性率6.58%,主侧阳性率2.47%,次侧阳性率4.12%,均显著高于凝聚胺法的1.65%、0.62%、1.03%(P<0.05);微柱凝胶法阳性病例检出不规则抗体阳性15例(主侧11例,次侧4例),凝聚胺法检出不规则抗体2例,两种方法阳性标本不规则抗体阳性检测结果差异无统计学意义(P>0.05);两种方法配血不合病因对比差异无统计学意义(P>0.05)。结论抗人球蛋白微柱凝胶法因其灵敏度高、操作便捷、重复性好、结果可靠,应成为交叉配血的首选方法,而凝聚胺法因其节省时间,在抢救用血中是必不可少的方法,两者可结合使用。 Objective To comparatively analyze results of anti-human globulin microcolumn gel technique and manual polybrene test in blood cross matching.Methods A total of 486 patients who were admitted to the hospital from January to December of 2018 and needed blood transfusion therapy were selected as the research subjects.Blood cross matching was conducted by anti-human globulin microcolumn gel technique or manual polybrene test.The results of these two blood cross matching methods were compared,and the causes of cross-matching incompatibility were analyzed.Results The duration of blood cross matching by microcolumn gel technique was significantly longer than by manual polybrene test(P<0.05).The total positive rate,primary positive rate and secondary positive rate of microcolumn gel technique(6.58%,2.47%and 4.12%)were significantly higher than those of the manual polybrene test(1.65%,0.62%and 1.03%)(P<0.05).In microcolumn gel technique positive cases,there were 15 irregular antibody positive cases(11 cases on the primary side and 4 cases on the secondary side),while 2 irregular antibody positive cases were detected by manual polybrene test.There was no significant difference in the positive detection results of irregular antibodies in samples with positive results between the two methods(P>0.05).There was also no significant difference in the causes of cross-matching incompatibility between the two methods(P>0.05).Conclusion The anti-human globulin microcolumn gel technique should be the preferred method for cross-matching because of its high sensitivity,convenient operation procedures,good reproducibility and reliable results.On the other hand,the manual polybrene test has the advantage of being an essential time-saving method in blood transfusion for rescue.In summary,the two methods can be used in combination.
