摘要
目的探究活血止痛胶囊对胫腓骨骨折(TF)患者术后疼痛、血清骨代谢指标及凝血功能的影响。方法将2017年8月—2019年8月行手术治疗的TF患者200例依据随机数字表法均分为两组。对照组术后常规西医处理,观察组除常规西医处理外给予活血止痛胶囊口服,干预7 d。对两组疗效、安全性进行评价,测定记录术前、术后7 d视觉模拟评分(VAS),肿胀程度,血清骨代谢指标[骨钙素(BGP)、骨形态发生蛋白-2(BMP-2)、降钙素(CT)、1型前胶原羧基端肽(PICP)]及凝血功能指标[D-二聚体(D-D)、凝血酶原时间(PT)、纤维蛋白原(FIB)、活化部分凝血活酶时间(APTT)]。结果观察组治疗后总有效率为91.00%(91/100),高于对照组的74.00%(74/100),差异有统计学意义(P<0.05)。与术后1 d比较,两组术后7 d VAS评分、肿胀程度、D-D、FIB水平均明显降低,BGP、BMP-2、CT、PICP水平明显上升,PT、APTT明显延长,且观察组各指标均明显优于对照组,差异有统计学意义(P<0.05)。两组均无不良反应出现。结论活血止痛胶囊能明显减轻TF患者术后疼痛,改善骨代谢及凝血指标,增强疗效,安全可行。
Objective To explore the effects of Huoxue Zhitong Capsules on postoperative pain,serum bone metabolism indexes and blood coagulation function in patients with tibio and fibular fracture(TF).Methods 200 patients with TF who underwent surgery in the hospital from August 2017 to August 2019 were enrolled.They were divided into 2 groups according to random number table method.After surgery,control group was given routine western medicine treatment,while observation group was given oral Huoxue Zhitong capsule on basis of routine western medicine intervention.They were intervened for 7 d.The curative effect and safety of both groups were evaluated.Before and at 7 d after surgery,scores of visual analogue scale(VAS),swelling degree,serum bone metabolism indexes[osteocalcin(BGP),bone morphogenetic protein-2(BMP-2),calcitonin(CT),type 1 procollagen carboxy terminal peptide(PICP)]and coagulation function indexes[D-dimer(D-D),prothrombin time(PT),fibrinogen(FIB),activated part thromboplastin time(APTT)]were measured and recorded.Results The total response rate of observation group was higher than that of control group(91.00%vs 74.00%)(P<0.05).Compared with those before surgery,VAS score,swelling degree,levels of DD and FIB in both groups were significantly decreased at 7 d after surgery,levels of BGP,BMP-2,CT and PICP were significantly increased,and PT and APTT were significantly prolonged.The above indexes in observation group were significantly better than those in control group(P<0.05).There was no adverse reaction in either groups.Conclusion Huoxue Zhitong Capsule can significantly alleviate postoperative pain in TF patients,improve bone metabolism and coagulation indexes,enhance curative effect.It is safe and feasible.
