摘要
目的分析胫骨中远端关节外骨折采用微创经皮钢板固定术与交锁髓内钉的治疗效果差异。方法在该院骨科2017年7月—2019年1月期间收治的胫骨中远端关节外骨折患者中,选取符合实验要求患者130例,随机分为观察A组(n=65,行微创经皮钢板固定术)和观察B组(n=65,行交锁髓内钉术),对比两组治疗效果。结果观察A组患者手术时间、术中出血量、愈合时间、住院时间均少于观察B组,两组对比差异有统计学意义(P<0.05),观察组A组患者AOFAS评分、治疗费用和观察B组对比差异无统计学意义(P>0.05);观察A组治疗总有效率为95.38%,观察B组治疗总有效率为89.23%,两组对比差异有统计学意义(χ^2=8.735,P<0.05);观察A组总并发症发生率为6.15%,观察B组总并发症发生率为18.46%,两组对比差异有统计学意义(χ^2=11.391,P<0.05)。结论在治疗胫骨中远端关节外骨折,采用微创经皮钢板固定术与交锁髓内钉治疗后患者术后的膝关节功能评价相差不明显,但在患者症状允许情况下选择微创经皮钢板固定术治疗,治疗效果更好,患者术后恢复较快、并发症较小。
Objective To analyze the effect of minimally invasive percutaneous plate fixation and interlocking intramedullary nailing on extra-articular fractures of the middle and distal tibia. Methods Among the patients with extra-articular fractures of the middle and distal tibia treated in the department of orthopedics from July 2017 to January 2019, 130 patients who met the experimental requirements were selected and randomly divided into observation group A(n =65, minimally invasive classics skin plate fixation) and observation group B(n =65, interlocking intramedullary nailing), the treatment effects of the two groups were compared. Results The operation time,intraoperative blood loss, healing time, and hospitalization time of the observation group A were all shorter than those of the observation group B. There was a statistically significant difference between the two groups P <0.05). The observation group AOFAS score and treatment cost had no statistically significant difference in comparison with observation group B(P>0.05);the total effective rate of treatment in observation group A was 95.38%, and the total effective rate of treatment in observation group B was 89.23%(χ^2=8.735, P <0.05);the total complication rate in observation group A was 6.15%, and the total complication rate in observation group B was 18.46%, and the difference between the two groups was statistically significant(χ^2=11.391, P<0.05). Conclusion In the treatment of extra-articular fractures of the middle and distal tibia, there is no significant difference in knee function evaluation after minimally invasive percutaneous plate fixation and interlocking intramedullary nail surgery. The treatment with skin plate fixation has better treatment effect, and the patients recover faster after operation with less complications.
