椎-基底动脉狭窄患者认知功能改变的临床研究 被引量:2

Clinical study on cognitive function changes in patients with vertebra-basilar artery stenosis

摘要 目的探讨椎-基底动脉狭窄患者认知功能损伤的特点。方法选取2016年2月至2017年10月在解放军火箭军特色医学中心经CT血管成像技术(CT angiography,CTA)或数字减影血管造影(digital subtraction angiography,DSA)检查证实,椎-基底动脉狭窄程度>70%或闭塞,且无其他血管狭窄患者34例(椎-基底动脉狭窄组)和无血管狭窄患者38例(对照组)。利用简易智能精神状态量表(mini-metal state examination,MMSE)、蒙特利尔认知量表(MontrealCognitive Assessment,Mo CA)及多维心理测试网络平台进行认知功能评估,进而对比两组间认知功能变化特点。结果椎-基底动脉狭窄组Mo CA得分显著低于对照组(P<0.05);而组间MMSE得分差异无统计学意义(P>0.05);两组在多维心理评估中的数字数量大小比较、简单数值计算、词语辨析结果测验、数字序列推理、物体数量认知方面的差异均有统计学意义(P<0.05)。结论椎-基底动脉狭窄会引起患者认知功能障碍,主要表现在注意力、言语功能、计算力和视觉感知能力。Mo CA及多维心理评估可早期发现血管性认知功能障碍。 Objective To evaluate the characteristics of cognitive impairment in the patients with vertebra-basilar artery stenosis.Methods Thirty-four patients proved by CTA or DSA with vertebra-basilar artery stenosis(>70%)or occlusion and without other vascular stenosis and 38 patients without vascular stenosis(control group)were chosen for the study.In order to compare the characteristics of cognitive impairment between the two groups,MMSE,Mo CA of Beijing edition and the network of multi-dimensional psychological test were used to assess the cognition of the patients.Results The score of Mo CA in the patients with vertebra-basilar artery stenosis was significantly lower than that in the control group(P<0.05),and there was no significant difference in the score of MMSE between the two groups(P>0.05).There were significant differences between the two groups in the comparison of number and quantity(P<0.05),simple numerical calculation(P<0.05),words differentiation(P<0.05),numerical sequence reasoning(P<0.05),and the cognition of objects quantity(P<0.05)in the multi-dimensional psychological assessment.Conclusions Vertebra-basilar artery stenosis can cause cognitive impairment,mainly including attention,speech,numeracy and visual perception.Mo CA and multi-dimensional psychological assessment can detect VCI early.
