摘要
目的对应用带线锚钉结合张力带内固定治疗髌骨粉碎性骨折的临床效果进行研究。方法采集时间段为2017年4月—2019年6月方便选取于该院骨科接受手术治疗的髌骨粉碎性骨折患者80例,通过随机法分为40例对照组(以克氏针张力带钢丝内固定术治疗)与40例研究组(以带线锚钉结合张力带内固定术治疗),对比不同手术方法的实际应用效果。结果对照组膝关节活动度优良率为77.5%,研究组膝关节活动度优良率为95.0%;经过对比后,相较于对照组,研究组患者膝关节活动度优良率更好(χ^2=12.911,P<0.05)。对照组Lysholm膝关节评分为(74.51±9.44)分,骨折愈合时间为(2.6±1.2)月,研究组Lysholm膝关节评分为(85.29±9.37)分,骨折愈合时间为(1.1±0.7)月;相较于对照组,研究组患者Lysholm膝关节评分及骨折愈合时间改善效果更好(t=5.125、6.828,P<0.05)。对照组并发症发生率为12.5%,研究组并发症发生率为2.5%;相较于对照组,研究组并发症发生率更低(χ^2=7.207,P<0.05)。结论在髌骨粉碎性骨折临床治疗中,可通过带线锚钉结合张力带内固定术进行治疗,膝关节活动功能恢复效果更好,使骨折愈合时间缩短,减少并发症发生率,具备较高临床价值。
Objective To study the clinical effect of applying sling anchors and tension band internal fixation to treat comminuted patella fractures.Methods The collection period was from April 2017 to June 2019.80 patients with comminuted patella fractures who underwent surgical treatment at the hospital's orthopaedics department were convenient scelection randomly divided into 40 control groups(treated with Kirschner wire tension band wire internal fixation)And 40 cases of the study group(treated with threaded anchors and tension band internal fixation),compared the actual application effects of different surgical methods.Results The excellent rate of knee joint mobility in the control group was 77.5%,and the excellent rate of knee joint mobility in the study group was 95.0%.After comparison,compared with the control group,the excellent rate of knee joint mobility was better in the study group(χ^2=12.911,P<0.05).The Lysholm knee score of the control group was(74.51±9.44)points,and the fracture healing time was(2.6±1.2)month.The Lysholm knee score of the study group was(85.29±9.37)points,and the fracture healing time was(1.1±0.7)month.Compared with the control,The Lysholm knee joint score and fracture healing time were better in the group and study group(t=6.828,5.125,P<0.05).The complication rate in the control group was 12.5%,and the complication rate in the study group was 2.5%.Compared with the control group,the complication rate was lower in the study group and the difference between the groups was large(χ^2=7.207,P<0.05).Conclusion In the clinical treatment of comminuted patella fractures,it can be treated with a wire anchor and tension band internal fixation.The knee joint has a better function of restoring the movement of the knee joint,with clinical value.
