摘要
目的探讨失眠障碍(insomnia disorder, ID)患者失眠严重程度与焦虑、抑郁的相关性。方法 50例ID患者,行汉密尔顿焦虑量表(Hamilton Anxiety Scale, HAMA)及汉密尔顿抑郁量表(Hamilton Depression Scale, HAMD)评分,其中ID伴焦虑14例为焦虑组,ID伴抑郁13例为抑郁组,ID伴焦虑抑郁16例为焦虑抑郁组,ID未伴焦虑或抑郁7例为对照组。采用失眠严重程度指数(Insomnia Severity Index, ISI)评估4组失眠程度,比较4组HAMA、HAMD评分及ISI,采用Spearman法分析HAMA、HAMD评分与ISI的相关性。结果焦虑抑郁组HAMA评分[(20.13±10.40)分]、HAMD评分[(16.50±8.04)分]高于焦虑组[(12.50±9.25)、(5.43±1.34)分]、抑郁组[(12.23±2.86)、(5.38±1.45)分]和对照组[(2.71±1.38)、(3.43±2.64)分](P<0.05)。ISI在焦虑抑郁组(17.31±5.95)、焦虑组(15.71±6.21)、抑郁组(14.77±5.50)、对照组(14.00±2.08)比较差异无统计学意义(P>0.05)。焦虑组HAMA评分与ISI呈正相关(r=0.562,P=0.036)。结论 ID患者失眠严重程度与焦虑症状有关,对ID患者的临床治疗中应注重对焦虑症状的筛查与干预。
Objective To investigate the correlations of the severity of insomnia with anxiety and depression symptoms in patients with insomnia disorder(ID).Methods Fifty ID patients underwent Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)scoring,in which 14 patients were complicated with anxiety(anxiety group),13 patients were complicated with depression(depression group),16 patient were complicated with anxiety and depression(anxiety and depression group),and 7 patients were not complicated with anxiety and depression(control group).Insomnia Severity Index(ISI)was used to assess the insomnia degree,and the differences in the scores of HAMA,HAMD and ISI were compared among four groups.Spearman method was used to analyze the correlations of HAMA and HAMD scores with ISI score.Results The scores of HAMA and HAMD were higher in anxiety and depression group(20.13±10.40,16.50±8.04)than those in anxiety group(12.50±9.25,5.43±1.34),depression group(12.23±2.86,5.38±1.45)and control group(2.71±1.38,3.43±2.64)(P<0.05).ISI score showed no significant difference among anxiety and depression group(17.31±5.95),anxiety group(15.71±5.21),depression group(14.77±5.50)and control group(14.00±2.08)(P>0.05).The HAMA score was positively correlated with the ISI score in anxiety group(r=0.562,P=0.036).Conclusion The severity of insomnia is correlated with anxiety.The clinical treatment of ID patients should focus on screening and intervention of anxiety symptom.
