摘要
目的探讨额叶背外侧癫痫患者视频脑电图(VEEG)特点,以提高额叶背外侧癫痫的临床正确诊断率。方法回顾性收集自2017年10月至2019年10月在新疆医科大学第六附属医院神经内科行VEEG检查,均有临床发作的额叶背外侧癫痫患者15例,分析其临床症状及VEEG表现。结果15例额叶背外侧癫痫患者均临床发作,其中10例主要以简单运动(Simple Motor,SM)症状为主,5例除了SM外,伴复杂运动(Complex Motor,CM)症状,如:咀嚼,上肢节律性拍打、“抓握”样动作,口中发出“哼哼”声,反复摇头等。VEEG发作间期显示痫样放电局限于额叶为7例,同时可累及同侧颞区5例、可累及中央区3例。VEEG发作期显示8例以全导电压下降、额区低幅快活动为起始,7例以额区尖化θ波或δ波活动为起始表现。结论额叶背外侧癫痫在临床表现及发作间期或发作期EEG均有特殊表现,需引起临床注意。
Objective To investigate the characteristics of video electroencephalogram(VEEG)in patients with dorsolateral frontal lobe epilepsy in order to improve the clinical diagnosis rate.Methods Retrospective collection of VEEG in the Department of Neurology,the Sixth Affiliated Hospital of Xinjiang Medical University from October 2017 to October 2019,15 patients with dorsolateral frontal lobe epilepsy who had clinical attacks were analyzed.Results All 15 patients with dorsolateral frontal lobe epilepsy had clinical seizures.Among them,10 were mainly with Simple Motor(SM)symptoms,and 5 were with SM and Complex Motor(CM)symptoms,such as chewing,Rhythmic tapping and"grasping"motions of upper limbs,"humming"sounds in the mouth,shaking his head repeatedly and so on.VEEG interictal period showed that epileptiform discharge was limited to 7 cases in frontal lobe,5 cases in ipsilateral temporal region and 3 cases in central region.VEEG onset period showed that 8 cases started with full conduction voltage drop,low amplitude fast activity in frontal region,and 7 cases started withθorδwave activity in frontal region.Conclusion Dorsolateral frontal lobe epilepsy has special manifestations in clinical manifestations and EEG interictal or seizure stages,which need to attract clinical attention.
