摘要
目的了解四川省临床营养发展现状,分析现阶段存在的主要问题,为制定临床营养工作管理规范提供依据。方法采用问卷对四川省部分二级以上综合或专科医院进行调查,用SPSS进行描述性分析。结果 78家医院参与调查,其中主要以三甲医院、地市级医院、综合医院、公立医院为主;营养科归口以医疗为主;营养科专业技术人员平均人数2人,本科/大专为主(68.0%),医师/技师为主(65.0%),初/中级为主(57.4%),平均营养师与床位比1∶313。营养科全年平均门诊人数888人,平均会诊人数1 011人,平均查房/随访人数3 912人;大部分医院硬软件设施不齐全;诊疗收费项目错综复杂,收费标准参差不齐,仍有46.5%的医院未开设诊疗收费项目。结论制约营养科发展的第一因素是人才队伍,希望未来全面加强人才队伍建设,开展营养师规范化培训,同时规范临床诊疗收费项目,推动营养科信息化建设。
Objective To figure out the current status of clinical nutrition in Sichuan Province, analyze the main problems existing at this stage, and provide the basis for formulating the management norms of clinical nutrition work. Methods Questionnaires were used to investigate secondary and tertiary general or specialized hospitals in Sichuan Province, and descriptive analysis was conducted with SPSS. Results A total of 78 hospitals were included in the survey, mainly in the tertiary hospitals, prefectural and municipal hospitals, general hospitals and public hospitals;the nutrition department mainly focused on medical treatment;the average number of professional and technical personnel in nutrition department was 2, with undergraduate/College(68.0%), physician/Technician(65.0%), junior/intermediate(57.4%), and the average ratio of nutritionists to beds was 1∶313. Nutrition department treated an average of 888 outpatient visits, 1011 consulting visits and 3912 follow-up visits. Most hospitals had a shortage of hardware and software facilities. The charged items were complex and the standards uneven. 46.5% of hospitals haven’t yet set up standards for charged items. Conclusion The primary factor restricting hospital development is the talent team. It is hoped that, in the future, the talent team construction can be comprehensively strengthened, standardized training for dietitians be carried out, charged clinical diagnosis and treatment items be standardized, and the informatization of the nutrition department be enhanced.
