摘要
目的观察安神热奄包联合血液透析滤过(HDF)对睡眠障碍血液透析患者睡眠质量、透析质量及炎症标志物超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)的影响。方法 60例行维持性血液透析治疗同时存在睡眠障碍的患者按随机数字表法分为研究组和对照组,各30例。对照组给予基础血液净化治疗和心理护理。研究组每周进行3次HDF治疗及3次安神热奄包足浴(足浴均在透析当天晚上进行),并给予心理护理。2组患者均治疗2个月。比较2组患者干预前后的匹兹堡睡眠质量指数评分(PSQI)、尿素清除指数(Kt/V)、炎症标志物水平。结果干预前,2组患者的PSQI评分、Kt/V数值、hs-CRP水平、IL-6水平、TNF-α水平比较差异均无统计学意义(P>0.05)。干预后,2组患者PSQI评分均较干预前降低(P<0.05),Kt/V数值均较干预前提高(P<0.05),血清hs-CRP、IL-6、TNF-α水平均较干预前降低(P<0.05);且研究组PSQI评分低于对照组(P<0.05),Kt/V数值高于对照组(P<0.05),血清hs-CRP、IL-6、TNF-α水平均低于对照组(P<0.05)。结论安神热奄包联合HDF可以明显改善血液透析患者的睡眠质量、透析质量和免疫微炎症状态。
Objective To observe the effect of foot bath therapy with Traditional Chinese Medicine(TCM)hot pack combined with hemodiafiltration on the sleep quality, dialysis quality, levels of hypersensitive c-reactive protein(hs-CRP), interleukin-6(IL-6)and tumor necrosis factor alpha(TNF-α)in hemodialysis patients with sleep disorders. Methods Totally 60 patients with maintenance hemodialysis and sleep disorders were randomly divided into study group and control group, with 30 cases in each group. The control group was given basic blood purification treatment(hemodiafiltration once per week and hematodialysis twice per week) and psychological nursing care. The study group was given hemodiafiltration three times per week and psychological nursing care, and foot bath therapy with TCM hot pack was carried out along with the treatment. The Pittsburgh Sleep Quality Index(PSQI), Urea clearance index(Kt/V) and serum levels of Inflammatory markers were compared between two groups. Results There was no significant difference in PSQI score, Kt/V, serum levels of hs-CRP, IL-6 and TNF-α between two groups before the intervention(P>0.05). After intervention, the Kt/V value in both groups was higher than that before intervention(P<0.05), and the PSQI score, levels of hs-CRP, IL-6 and TNF-αin both groups were lower than that before intervention(P<0.05). After intervention, Kt/V value of the study group was higher than that of the control group(P<0.05);the PSQI score, levels of hs-CRP, IL-6 and TNF-α of the study group were lower than that of the control group(P<0.05). Conclusion The combination of foot bath therapy with Traditional Chinese Medicine hot pack and hemodiafiltration can improve the sleep quality, dialysis quality and immune function in hemodialysis patients.
