踝臂指数、脂蛋白相关磷脂酶A2、缺血修饰白蛋白与腹膜透析患者心血管事件的相关性分析 被引量:1

摘要 目的:探讨踝臂指数(ABI)、脂蛋白相关磷脂酶A2(Lp-PLA2)、缺血修饰白蛋白(IMA)对腹膜透析(PD)患者心血管事件(MACE)的影响,并分析PD患者高ABI的影响因素。方法:选择2017年10月-2019年2月于笔者所在医院行PD的108例患者作为研究对象,根据ABI水平将其分为研究组(ABI>1.3,42例)与对照组(1.0<ABI<1.3,66例),以患者死亡或发生MACE为观察终点。比较两组临床资料及MACE发生情况,采用Spearman分析ABI及各监测指标与MACE的相关性,采用Logistic回归分析高ABI的独立危险因素。结果:研究组发生MACE 20例(47.6%),显著高于对照组的12例(18.2%),差异有统计学意义(P<0.05)。研究组透析龄、血压、血钙、血磷、Lp-PLA2、PTH、hs-CRP、IMA均显著高于对照组,差异有统计学意义(P<0.05)。Spearma分析显示,ABI、Lp-PLA2、IMA、透析龄、血钙、PTH、hs-CRP均与MACE呈正相关(P<0.05)。Logistic回归分析显示,血钙、透析龄、hs-CRP、IMA与Lp-PLA2是PD患者高ABI的独立危险因素。结论:PD患者ABI与MACE具有正相关性,对于早期预测MACE具有重要的指导价值,IMA与Lp-PLA2是影响ABI升高的因素,且与MACE具有显著正相关性,临床应密切关注。 Objective:To investigate the influence of ankle-brachial index(ABI),lipoprotein-associated phospholipase A2(Lp-PLA2)and ischemia modified albumin(IMA)on major adverse cardiovascular events(MACE)in patients with peritoneal dialysis(PD),and to analyze the influencing factors of high ABI in PD patients.Method:A total of 108 patients who received PD in our hospital from October 2017 to February 2019 were selected as the research objects.According to the ABI level,they were divided into the study group(ABI>1.3,42 cases)and the control group(1.0<ABI<1.3,66 cases).The death or occurrence of MACE was considered as the observation endpoint.The clinical data and the occurrence of MACE were compared between the two groups.Spearman was used to analyze the correlation between ABI and each monitoring index and MACE,and Logistic regression was used to analyze the independent risk factors of high ABI.Result:MACE occurred in 20 cases(47.6%)in the study group,which was significantly higher than 12 cases(18.2%)in the control group,the difference was statistically significant(P<0.05).The dialysis age,blood pressure,blood calcium,blood phosphorus,Lp-PLA2,PTH,hs-CRP,and IMA in the study group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).Spearma analysis showed that ABI,Lp-PLA2,IMA,dialysis age,blood calcium,PTH,hs-CRP were positively correlated with MACE(P<0.05).Logistic regression analysis showed that blood calcium,dialysis age,hs-CRP,IMA,and Lp-PLA2 were independent risk factors for high ABI in PD patients.Conclusion:ABI is positively correlated with MACE in PD patients,which has important guiding value for early prediction of MACE.IMA and Lp-PLA2 are factors affecting the rise of ABI,and have significant positive correlation with MACE,which should be paid close attention to clinically.
