摘要
目的:探讨胆囊部分切除术治疗胆囊炎合并胆结石的临床效果。方法:选取2017年1月-2020年1月笔者所在医院收治的胆囊炎合并胆结石患者72例,随机分为两组,对照组采用胆囊全切除术,研究组采用胆囊部分切除术,对比两组手术时间、术中出血量、术后引流量、术后首次排气时间、临床效果、并发症发生率。结果:研究组术中出血量为(37.2±5.3)ml,术后引流量为(27.9±6.8)ml,均少于对照组的(62.3±10.2)、(50.2±7.9)ml,差异均有统计学意义(P<0.05);研究组手术时间为(65.3±5.0)min,术后首次排气时间为(1.1±0.3)d,均短于对照组的(82.9±6.0)min、(2.5±0.4)d,差异均有统计学意义(P<0.05);研究组总有效率为96.67%,高于对照组的77.78%,差异有统计学意义(χ^2=6.222,P<0.05);研究组并发症发生率为5.56%,低于对照组的25.00%,差异有统计学意义(χ^2=5.258,P<0.05)。结论:将胆囊部分切除术应用于胆囊炎合并胆结石的临床治疗中,可缩短手术时间与术后首次排气时间,减少术中出血量与术后引流量,降低并发症发生率,临床效果显著,值得推广应用。
Objective:To investigate the clinical effect of partial cholecystectomy for cholecystitis complicated with gallstones.Method:From January 2017 to January 2020,72 patients with cholecystitis complicated with gallstones admitted to our hospital were selected and randomly divided into two groups,the control group was treated with total cholecystectomy,and the study group was treated with partial cholecystectomy.Operation time,amount of intraoperative bleeding,postoperative drainage volume,postoperative first exhaust time,clinical effect and incidence of complications were compared between the two groups.Result:The amount of intraoperative bleeding in the study group was(37.2±5.3)ml,and the postoperative drainage volume was(27.9±6.8)ml,which were less than(62.3±10.2)and(50.2±7.9)ml of the control group,and the differences were statistically significant(P<0.05).The operative time of the study group was(65.3±5.0)min,and the postoperative first exhaust time was(1.1±0.3)d,which were shorter than(82.9±6.0)min,(2.5±0.4)d of the control group,and the differences were statistically significant(P<0.05).The total effective rate of the study group was 96.67%,which was higher than 77.78%of the control group,and the difference was statistically significant(χ^2=6.222,P<0.05).The incidence of complications in the study group was 5.56%,which was lower than 25.00%of the control group,and the difference was statistically significant(χ^2=5.258,P<0.05).Conclusion:The application of partial cholecystectomy in the clinical treatment of cholecystitis complicated with gallstones can shorten the operation time and postoperative first exhaust time,reduce amount of intraoperative bleeding and postoperative drainage volume,and reduce the incidence of complications.The clinical effect is significant,and it is worthy of popularization and application.
