摘要
目的探讨原发性高血压患者25羟基维生素D[25(OH)D]水平与血浆肾素浓度及尿酸的相关性。方法选择河南省人民医院确诊的原发性高血压患者510例。采用电化学发光法测血清25(OH)D水平。根据25(OH)D水平将患者分为缺乏组(n=408)和非缺乏组(n=102)。采用化学发光法直接测定血浆肾素浓度。全自动生化学分析仪检测生化全项,包括血尿酸。结果维生素D缺乏组的血尿酸值高于维生素D非缺乏组[340(292~404)比237(223~371)μmol/L,P<0.05],立位肾素浓度高于维生素D非缺乏组[17.43(8.81~34.97)比10.04(4.26~19.49)pg/mL,P<0.05]。Spearman相关分析提示,25(OH)D水平与立位肾素浓度、血尿酸呈负相关(分别为r=-0.322,P<0.001;-0.210,P<0.001)。多元线性逐步回归显示,25(OH)D、24 h平均心率、年龄是立位肾素浓度的独立相关因素(β值分别为-0.282、0.165、-0.100,P<0.05),25(OH)D、饮酒、平均SBP是血尿酸独立相关因素(β值分别为-0.22,0.199,0.092,P<0.05);按年龄分层后,25(OH)D仍是两者的独立相关因素。结论原发性高血压患者伴维生素D缺乏者立位肾素浓度、血尿酸高于维生素D非缺乏组,25(OH)D是立位肾素浓度及血尿酸的独立相关因素。
Objective To investigate the relationship between 25-hydroxyvitamin D[25(OH)D]with plasma renin concentration and uric acid in patients with essential hypertension(EH).Methods A total of 510 EH patients in Henan Province People's Hospital 2015-2017 were recruited and divided into vitamin D deficiency group[25(OH)D<50 nmol/L(20 ng/mL),n=408]and non-deficiency group[25(OH)D≥50 nmol/L,n=102]according to 25(OH)D measured by electrochemiluminescence.Plasma renin concentration was measured by chemiluminescence.Automatic biochemical analyzer was used to test the biochemical items including uric acid.Results Compared with vitamin D non-deficiency group,the uric acid and plasma renin concentration were both higher in deficiency group[uric acid:340(291.5~404)vs 237(223~371)μmol/L;17.43(8.81~34.97)vs 10.04(4.26~19.49)pg/mL,all P<0.05].Spearman correlation analysis showed that 25(OH)D was negatively correlated with plasma renin concentration and uric acid(r=-0.322,P<0.001;-0.210,P<0.001 respectively).Multivariate liner analysis showed that 25(OH)D,24 h mean heart rate and age were independent influence factors of plasma renin concentration(β=-0.282,0.165,-0.100 respectively,P<0.05);25(OH)D;drinking and SBP were independent factors of uric acid(β=-0.22,0.199,0.092 respectively,all P<0.05).Conclusion Plasma renin concentration and uric acid in patients with vitamin D deficiency of EH are higher than that in vitamin D non-deficiency group.25(OH)D is an independent correlation factor of plasma renin concentration and uric acid。
