阿卡贝拉训练仪在肺癌术后快速康复中的作用 被引量:9

Application effect of Acapella in rapid rehabilitation of lung cancer after operation

摘要 目的:探讨阿卡贝拉(Acapella)训练仪在肺癌术后快速康复中的应用效果,为肺癌术后患者提高生存质量提供帮助。方法:选取2018年8月至2019年2月在江苏省肿瘤医院胸外科行全电视胸腔镜(VATS)肺癌手术治疗的患者共60例作为本次研究对象,根据数字表法随机分为两组:观察组( n=30)和对照组( n=30)。对照组患者术后采用常规肺康复锻炼,观察组患者在此基础上加用Acapella治疗仪进行术后康复。观察并比较两组患者使用Acapella前后主要肺功能指标,主要为用力肺活量(FVC)、第一秒用力呼气量(FEV1)、呼气流量峰值(PEF)、术后咳痰难易度、痰液黏稠度、24 h胸管引流量、住院时间及6 min步行距离(6MWD)等指标变化。应用SPSS 19.0统计软件分析,组间比较采用 χ2检验、 t检验,以 P<0.05为差异有统计学意义。 结果:使用Acapella治疗后,观察组患者FEV1提高幅度]优于对照组[(2.51±0.68) L比(2.53±0.59) L, t=-2.714, P<0.05];观察组患者FVC高于对照组[(2.97±0.70) L比(2.61±0.65) L, t=-2.045, P<0.05];观察组患者PEF高于对照组[(4.84±1.66) L比(4.43±1.57) L, t=-0.707, P<0.05];观察组患者6MWD长于对照组[(593.20±4.76) m比(532.97±9.83) m, t=-5.514, P<0.05],其差异均有统计学意义。临床指标方面,观察组术后咳痰难易度与对照组相比更易自行咳痰(1.07±0.25比1.30±0.47, t=7.449, P<0.05);观察组术后痰液粘稠度与对照组相比更易排出(1.03±0.18比1.23±0.43, t=10.428, P<0.05);观察组胸管置管时间与对照组相比明显缩短[(5.33±1.73) d比(6.67±2.26) d, t=2.564, P<0.05];观察组24 h引流量与对照组相比明显减少[(10.50±4.22) ml比(13.50±6.72) ml, t=13.042, P<0.05];观察组住院时间与对照组相比明显缩短[(12.07±3.48) d比(15.37±7.16) d, t=2.270, P<0.05],差异均有统计学意义。 结论:对肺癌术后患者在肺康复锻炼基础上加用Acapella治疗,能够改善患者肺功能,同时促使患者排痰,缩短住院时间,有助于肺癌患者术后生活质量提高。 Objective To investigate the application effect of Acapella in the rapid rehabilitation of lung cancer after operation.Methods Sixty patients treated with VATS for lung cancer at Jiangsu Cancer Hospital from August 2018 to February 2019 were enrolled and randomly divided into 2 groups:observation group(n=30)and control group(n=30).All patients were treated with routine lung recovery nursing after VATS,while Acapella was added to the patients in observation group.The changes of forced vital capacity(FVC),forced expiratory volume in one second(FEV1),peak expiratory flow(PEF),facility value and property of sputum expectoration,chest drainage,postoperative hospital stay and 6-minute walking distance(6MWD)were observed and a comparative study was made between the 2 groups.Results After using Acapella,alteration of FEV1 in observation group[(2.51±0.68)L]was higher than that in control group[(2.53±0.59)L,t=-2.714,P<0.05];the FVC in observation group[(2.97±0.70)L]was higher than that in control group[(2.61±0.65)L,t=-2.045,P<0.05];the PEF in observation group[(4.84±1.66)L]was higher than that in control group[(4.43±1.57)L,t=-0.707,P<0.05];and the 6MWD in observation group[(593.20±4.76)m]was longer than that in control group[(532.97±9.83)m,t=-5.514,P<0.05].After using Acapella,the facility value of sputum expectoration in observation group(1.07±0.25)was easier than that in control group(1.30±0.47,t=7.449,P<0.05);the property of sputum expectoration in observation group(1.03±0.18)was lower than that in control group(1.23±0.43,t=10.428,P<0.05).Moreover,time of chest drainage in observation group[(5.33±1.73)d]was shorter than that in control group[(6.67d±2.26)d,t=2.564,P<0.05];the volume of daily chest drainage in observation group[(10.50±4.22)ml]was less than that in control group[(13.50±6.72)ml,t=13.042,P<0.05];and the postoperative hospital stay in observation group[(12.07±3.48)d]was shorter than that in control group[(15.37±7.16)d,t=2.270,P<0.05],the difference was statistically significant.Conclusion Acapella could improve the patients’respiratory function,meanwhile improving the quality of life.Acapella is of vital curative effect on patients with VATS postoperative lung cancer.
