摘要
目的了解耐多药肺结核病人治疗过程中甲状腺功能减退影响因素,寻求抗结核药物导致的甲状腺功能减退临床表现和治疗方法,避免甲状腺功能减退症的发生。方法对耐多药结核病人性别、年龄、结核病史详细记录;观察病人临床表现、实验室检查及治疗情况。结果结核药物引起甲状腺功能减退和性别、年龄、结核病史无密切关系,差异无统计学意义(P>0.05)。耐多药肺结核药物引起甲状腺功能减退临床表现不典型且出现较晚,化验检查异常出现异常滞后。结论抗结核药丙硫异烟胺与对氨基水杨酸钠联合用药可以引起甲状腺功能减退,临床表现不典型,易漏诊,应引起结核科临床大夫的注意。
Objective To understand the influencing factors of hypothyroidism in patients with multidrug-resistant tuberculosis(MDR-TB),and to seek the clinical manifestations and treatment of hypothyroidism caused by anti-TB drugs in order to avoid hypothyroidism.Methods The data of patients with MDR-TB were recorded in detail,including gender,age and TB history.Then the clinical manifestations of the patients,laboratory examination and treatment were observed.Results The hypothyroidism caused by TB drugs was not closely related to sex,age or history of TB.There was no statistically significant difference(P>0.05).The clinical manifestations of hypothyroidism caused by MDR-TB drugs were atypical and appeared late,and there was abnormal lag in laboratory examination.Conclusion The protionamide,one kind of anti-TB drugs,combined with para-amino salicylate can cause hypothyroidism with atypical clinical manifestations.Because of the easy misdiagnosis of hypothyroidism,the clinicians in the tuberculosis department should pay more attention to it.
