摘要
目的探讨黏液纤维肉瘤(Myxofibrosarcoma,MFS)的CT/MRI表现,并与病理结果对照。方法回顾性分析经手术病理证实的10例MFS患者的临床、影像资料及病理结果。结果10例MFS主要位于四肢、臀部和髂窝,位置深在或表浅,病灶长径平均10.78cm,9例边界不清或局部欠清。CT平扫呈低或稍低密度;8例MRI检查者6例见“尾征”,7例见低信号线样分隔。T1WI以等或低信号为主,2例伴有条片状稍高信号,T2WI以高信号为主,5例信号混杂。DWI显示弥散明显不均匀受限,增强扫描强化明显均匀或不均匀。镜下见黏液基质背景中分布数量不等的梭形肿瘤细胞,高级别者异型性和核分裂象明显。结论MFS富含黏液成分,部分病例可见“尾征”,具有一定特征性。MRI在反映病灶内成分方面优于CT,对MFS的诊断具有重要价值。
Objective To investigate the correlation between the imaging features and histopathology of myxofibrosarcoma(MFS).Methods Ten cases with MFS confirmed by pathology were enrolled.The clinical materials,imaging and pathological findings were retrospectively analyzed.Results The tumors were mainly located at the limbs,buttocks and iliac fossa,deep-seated or superficial.The average long dimension was 10.78 cm,and 9 cases of them were ill-defined.CT plain scan showed hypo-attenuated or slightly hypo-attenuated density.6 of the 8 patients who underwent MRI examination showed the“tail”sign,and 7 cases showed linear hypointense separation.On T1WI,most cases showed isointense or hypointense,with 2 cases showing flaky slightly higher signal.On T2WI,most cases showed hyperintense,with 5 cases showing mixed signal.DWI(b=800 s/mm^2)presented mixed high signal,and enhanced MRI showed obviously homogeneous enhancement or obviously inhomogeneous enhancement.The histopathological feature of MFS showed that a large number of spindle tumor cells were distributed in the myxoid matrix background,with more obvious atypia and mitosis in higher grade MFS.Conclusion MFS shows some characteristic imaging features on MRI,such as“tail sign”and the mucous component within the lesion.In addition,MRI is superior to CT in reflecting the components within the lesion,which is of vital importance to diagnosis.
