摘要 目的探讨不同滤除白细胞方法对悬浮红细胞多项指标的影响。方法选取2017年4月至2019年5月本血站临床使用的560袋悬浮红细胞,随机分为洗涤法滤白组和过滤法滤白组各280袋,分别采用洗涤法和过滤法进行白细胞滤除,检测滤除前后悬浮红细胞(RBC)畸变率、红细胞比容、游离血红蛋白(Hb)、Hb、RBC溶血率以及P50指标变化情况。结果两组滤白前RBC畸变率和红细胞比容对比差异无统计学意义(P>0.05);滤白后两组RBC畸变率均升高,但洗涤法滤白组滤白前后RBC畸变率比较差异无统计学意义(P>0.05),过滤法滤白组滤白后RBC畸变率明显高于滤白前(P<0.05),滤白后洗涤法滤白组RBC畸变率低于过滤法滤白组(P<0.05);滤白后两组红细胞比容均降低,但差异无统计学意义(P>0.05)。两组滤白前游离Hb、Hb比较差异无统计学意义(P>0.05);两组滤白后游离Hb均有升高(P<0.05);滤白后两组Hb均降低,且洗涤法滤白组低于过滤法滤白组(P<0.05),两组滤白前RBC溶血率、P50比较差异无统计学意义(P>0.05);两组滤白后RBC溶血率均升高,P50均下降,但与滤白前比较差异无统计学意义(P>0.05)。结论两种不同滤白方法均能很好地实现白细胞滤除效果,但过滤法会增加RBC畸变率,虽然不会影响血液使用安全,但临床滤除白细胞最好结合实际输血需求选择有效方法。 Objective To explore the effects of different methods of leukocyte removal on several indices of suspended red blood cells. Methods From April 2017 to may 2019, 560 bags of suspended red blood cells were selected from the central blood station of Huizhou, and randomly divided into two groups, with 280 bags in each group. White blood cell was filtrated by washing and filtration, respectively. RBC distortion rate, RBC specific volume, free Hb, Hb, RBC hemolysis rate and P50 index were measured before and after filtration. Results There was no significant difference in RBC aberration rate and RBC specific volume between the two groups before filtering(P>0.05). After filtering, the RBC aberration rate increased in both groups, but there was no difference in RBC aberration rate between pre-and post-bleaching groups(P>0.05). The RBC aberration rate of the washing group was significantly higher than that of the filtration group(P<0.05). The RBC aberration rate of washing group was lower than that of filtration group(P <0.05). After bleaching, the specific volume of erythrocyte decreased in both groups, but there was no significant difference(P>0.05). There was no significant difference in free Hb and Hb between the two groups before filtering(P>0.05). Free Hb increased after bleaching in both groups, there was significant difference between the two groups(P<0.05). After bleaching, Hb decreased in both groups, the washing method was lower than the filtering method(P<0.05). There was no significant difference in RBC hemolysis rate and P50 between the two groups before bleaching(P>0.05). The hemolysis rate of RBC increased and P50 decreased after bleaching in both groups, however, there was no significant difference between the two groups(P>0.05). Conclusion Two different methods of white blood cell filtration can achieve good results, but the filtering method can increase the distortion rate of RBC, although it does not affect the safety of blood use. However, clinical leukocyte removal is best combined with the actual needs of blood transfusion to select effective methods.
