摘要
目的应用薏苡仁油联合顺铂腹腔注射治疗结直肠癌恶性腹腔积液,观察临床疗效。方法将60例确诊结直肠癌恶性腹腔积液患者随机分为试验组与对照组,每组30例。试验组患者应用薏苡仁油联合顺铂进行腹腔热灌注治疗;对照组单纯顺铂腹腔热灌注治疗。治疗1个月后观察2组患者治疗前后腹水情况、患者生活质量、不良反应发生率等情况;检测治疗前及治疗1个月后外周血TBX4的mRNA、肿瘤坏死因子-α(TNF-α)、γ-干扰素(IFN-γ)、超氧化物歧化酶(SOD)、丙二醛(MDA)水平进行比较。结果试验组与对照组的有效率分别为83.33%、60.00%,试验组患者治疗有效率高于对照组(χ^2=4.022,P=0.045);灌注后试验组与对照组患者生活质量综合评分的改善率分别为76.67%、50.00%,试验组明显高于对照组(χ^2=4.593,P=0.032);试验组恶心、呕吐,骨髓抑制,肝功能损害的发生率明显低于对照组(χ^2=4.812,P=0.028;χ^2=4.022,P=0.045;χ^2=4.320,P=0.038),而腹痛、腹胀发生率与对照组差异无统计学意义(P>0.05)。2组患者治疗后外周血TBX4的mRNA、IFN-γ、SOD比治疗前升高,试验组升高更为明显(t=2.629,P=0.036;t=2.814,P=0.031;t=3.105,P=0.021);2组患者治疗后血浆TNF-α、MDA水平均比治疗前下降,试验组下降更为明显(t=-2.614,P=0.040;t=-2.884,P=0.028)。结论薏苡仁油联合顺铂腹腔注射治疗结直肠癌恶性腹水有较好的效果。
Objective To investigate the therapeutic effects of Coix seed oil combined with cisplatin intraperitoneal injection in treatment of malignant seroperitoneum in patients with colorectal cancer.Methods A total 60 patients with malignant seroperitoneum due to colorectal cancer were randomly divided into experimental group and control group,with 30 cases in each group.The patients in experimental group were treated by Coix seed oil combined with cisplatin,however,the patients in control group were treated by simple cisplatin.At one month after treatment,the ascites status,quality of life and incidence of side effects were observed in both groups.Moreover the levels of TBX4,TNF-α,IFN-γ,SOD and MDA in peripheral blood before treatment and at one month after treatment were observed and compared between the two groups.Results The effective rate in experimental group was 83.33%,which was significantly higher than that(60.00%)in control group(P<0.05).After perfusion therapy,the improvement rate of comprehensive life quality score in experimental group was 76.67%,which was significantly higher than that(50.00%)in control group(P<0.05).The incidence rates of nausea,vomiting,bone marrow suppression and liver function damage in experimental group were significantly lower than those in control group(P<0.05),however,there were no significant differences in the incidence rates of abdominal pain and distention between the two groups(P>0.05).After treatment,the levels of TBX4 gene,IFN-γand SOD in peripheral blood of both groups were significantly increased,as compared with those before treatment,moreover,the increase degree in experimental group was more significant than that in control group(P<0.05).In addition the levels of TNF-αand MDA in both groups were decreased after treatment,moreover,the decrease degree in experimental group was more significant than that in control group(P<0.05).Conclusion Coix seed oil combined with cisplatin intraperitoneal injection has better therapeutic effects on malignant ascites due to colorectal cancer.
