摘要
目的研究开放式牙槽嵴保存术对磨牙拔除位点的影响。方法选择2017年7月~2018年6月于我院口腔科就诊的因牙周疾病需拔除磨牙的患者,按照给予牙槽嵴保存方法不同分为联合组6例、单纯骨移植组7例及对照组7例。联合组给予去蛋白牛骨基质Bio-Oss骨粉与双层可吸收天然胶原蛋白膜(Bio-Gide)保存牙槽嵴,单纯骨移植组仅给予Bio-Oss骨粉,对照组自然愈合。采用CBCT扫描对术前、术后6个月牙槽嵴根方1mm、3mm、5mm处水平牙槽嵴宽度,颊侧、舌侧区垂直牙槽嵴高度。结果术后6个月联合组△W5明显高于单纯骨移植组(P<0.05);术后6个月联合组△W1、△W3明显高于对照组(P<0.05)。结论采用Bio-Oss骨粉与双层Bio-Gide进行牙槽嵴保存可有效减少牙槽嵴宽度及高度的吸收程度,利于后续种植修复术的进行。
Objective To investigate the effect of alveolar ridge preservation on molar extraction sites with an open healing approach.Methods 21 patients with periodontal disease requiring extraction of molars(21 sites)in our hospital from July 2017 to June 2018 were enrolled.Patients were randomly divided into combined group,bone graft group and control group,each with 7 teeth.Combined group was grafted with deproteinized bovine bone mineral matrix(Bio-Oss powder)and covered by Bio-Gide resorbable collagen membrane,while bone graft group only received the Bio-Oss powder.The control group received no treatment and healed naturally.Horizontal ridge changes at 1,3,and 5 mm below the crest,and the changes in the vertical heights of the ridge at the buccal crest and lingual crest were detected by cone beam computed tomography(CBCT)images before and 6mon after operation.Results△W5 at postoperative 6mon was higher in the combined group than in the bone graft group(P<0.05).△W1 and△W3 at postoperative 6mon were higher in the combined group than in control group(P<0.05).Conclusion Bio-Oss powder combined with double-layer Bio-Gide can effectively reduce the can effectively reduce the degree of absorption of the alveolar ridge width and height,which is conducive to the subsequent implant restoration.
