摘要
目的探讨真武汤合血府逐瘀汤辅助治疗对慢性心力衰竭(CHF)患者心功能及血流动力学指标的影响。方法选取2019年3月至2020年1月瓦房店六二六中医医院有限公司中医内科收治CHF患者115例作为研究对象,随机分为对照组(n=57)和研究组(n=58),对照组患者给予常规西药治疗,研究组在对照组的基础上给予中药真武汤合血府逐瘀汤治疗,比较两组患者心功能、血流动力学指标及临床疗效。结果治疗后研究组患者心功能指标[心室收缩末期内径(LVESD)、左心室舒张末径(LVEDD)]低于对照组,而心输出量(CO)、左心室射血分数(LVEF)、舒张早期峰值速度(E值)/舒张晚期峰值速度(A值)高于对照组,差异有统计学意义(P<0.05);治疗后,研究组患者平均二尖瓣跨瓣压差(MMPG)、左心房平均压(MLAP)、肺动脉平均压(MPAP)明显低于对照组,差异有统计学意义(P<0.05);研究组患者治疗有效率显著高于对照组,差异有统计学意义(P<0.05)。结论在CHF患者的治疗过程中,真武汤合血府逐瘀汤与西药联合应用的临床效果优于单用西药治疗,患者心功能改善程度高,血流动力学稳定。
Objective To explore the effects of Zhenwu Decoction and Xuefu Zhuyu Decoction on cardiac function and hemodynamic parameters in patients with chronic heart failure. Methods A total of 115 patients with CHF admitted to TCM Internal Medicine Department, Zhenwu Decoction from March 2019 to January 2020 were selected as the research objects and were divided into a control group(n=57) and a study group(n=58). The patients in the control group were treated with conventional Western medicine, while those in the study group were treated with Zhenwu Decoction and Xuefu Zhuyu Decoction on the basis of the control group.The changes of cardiac function, hemodynamic parameters and clinical efficacy in the 2 groups were compared.Results After treatment, the indexes of cardiac function(LVESD, LVEDd, E/a) in the study group were lower than those before treatment, while CO and LVEF and early diastolic peak velocity(E value)/late diastolic peak velocity(A value) were higher than those in the control group before treatment, and the difference was statistically significant(P < 0.05). After treatment, the hemodynamic parameters such as MMPG, MLAP and MPAP in the study group were significantly lower than those in the control group, and the difference was statistically significant(P<0.05). The effective rate of the study group was significantly higher than that of the control group, and the difference was statistically significant(P<0.05). Conclusion In the treatment of CHF patients,the clinical effects of Zhenwu Decoction combined with Xuefu Zhuyu Decoction and Western medicine are better than that of Western medicine alone. The patients’ heart function is improved to a high degree and hemodynamic is stable.
