摘要
目的研究妊娠期女性体内甲状腺自身免疫标志物的水平。方法采用病例对照研究,方便收集自2016年1月—2019年12月在该院妇产科门诊就诊的患者。病例组选择诊断为低甲状腺素血症的妇女37名,对照组选择到该院进行正常孕检且甲状腺功能正常者53名,对比两组的体质指数(BMI)、甲状腺功能和血脂水平。结果病例组的BMI均值为29.5 kg/m^2,高于对照组的27.5 kg/m2(U=735.5,P<0.001),空腹葡萄糖、胰岛素抵抗稳态模型(HOMA-IR)、促甲状腺激素(TSH)水平、糖化血红蛋白(HbA1c)、甘油三酯(TC)、总胆固醇(TG)和低密度脂蛋白(LDL)水平分别为4.5 mmol/L、2.0、1.9 mIU/L、5.4%、2.3 mmol/L、3.7 mmol/L、3.1 mmol/L均显著高于对照组的4.3 mmol/L、1.4、1.5 mIU/L、4.9%、2.0 mmol/L、3.4 mmol/L、2.91 mmol/L(U=706.5、674.5、598.0、727.5、582.5、614.0、592.0;P=0.021、0.010、0.001、0.033、<0.001、0.002、<0.001)。对照组的游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)和高密度脂蛋白(HDL)水平分别为4.2 pmol/L、12.5 pmol/L和1.8 mmol/L均显著高于病例组的4.1 pmol/L、9.7 pmol/L和1.7 mmol/L(U=706.5、594.5、574.5;P=0.021、<0.001、<0.001)。结论妊娠低甲状腺素血症与孕妇的肥胖高度相关,临床应加以重视,给予孕产妇正确的饮食和治疗指导。
Objective To study the level of thyroid autoimmune markers in pregnant women.Methods A case-control study was convenient used to collect patients from the outpatient department of Obstetrics and Gynecology in the hospital from January 2016 to December 2019.The case group selected 37 women diagnosed with hypothyroxinemia,and the control group selected those who went to the hospital for normal pregnancy tests and 53 people had normal thyroid function.The body mass index(BMI),thyroid function and blood lipid levels of the two groups were compared.Results The mean BMI of the case group was 29.5 kg/m^2,higher than that of the control group27.5 kg/m2(U=735.5,P<0.001),fasting glucose,insulin resistance steady state model(HOMA-IR),thyroid stimulating hormone(TSH)level,glycated hemoglobin(HbA1c),Triglyceride(TC),total cholesterol(TG)and low density lipoprotein(LDL)levels were 4.5 mmol/L,2.0,1.9 mIU/L,5.4%,2.3 mmol/L,3.7 mmol/L,3.1 mmol/L were significantly higher than the control group’s 4.3 mmol/L,1.4%,1.5 mIU/L,4.9%,2.0 mmol/L,3.4 mmol/L,2.9 mmol/L(U=706.5,674.5,598.0,727.5,582.5,614.0,592.0;P=0.021,0.010,0.001,0.033,<0.001,0.002,<0.001).The levels of free triiodothyronine(FT3),free thyroxine(FT4)and high-density lipoprotein(HDL)in the control group were 4.2 pmol/L,12.5 pmol/L and 1.8 mmol/L,respectively,which were significantly higher than those in the case The group's 4.1 pmol/L,9.7 pmol/L and 1.7 mmol/L(U=706.5,594.5,574.5;P=0.021,<0.001,<0.001).Conclusion Hypothyroxinemia in pregnancy is highly correlated with obesity of pregnant women.It should be paid attention to clinically and give pregnant women correct diet and treatment guidance.
