摘要
目的探讨单导联可穿戴心电设备结合散点图在心律失常检出率上的应用,及其在“互联网+慢病管理”中的应用价值.方法选取2018年6月至2019年12月由银川市所属基层医疗机构辖区纳入的50岁以上有头晕、胸闷、心悸、气促症状的360例患者,将患有高血压、糖尿病、冠心病、慢性阻塞性肺疾病等慢性病的180例患者作为观察组,对照组为有上述症状但无慢性病的180例患者.所有受试者均在有上述症状时,自行佩戴单导联可穿戴心电设备进行24 h心电数据采集并实时上传到云端,基层医师依据散点图初步诊断,再由银川市第一人民医院远程心电诊断中心出具诊断报告.统计观察组与对照组心律失常检出率.结果心律失常总检出率为68.61%,观察组为89.44%,高于对照组的47.77%,差异有统计学意义(P<0.05).结论单导联可穿戴心电设备实用便携,提高了心律失常的检出率,与散点图结合并借助“互联网+”有利于慢性病患者的管理.
Objective To explore the possibility of the single-lead wearable ECG monitoring device to;increase the dection rate of arthythmia and its application value of combining with the scatterplot and"internet+"in the management of chronic disease.Methods 360 patients over 50 years old with symptoms of dizziness,chest tightness,palpitation and shortness of breath were selected from June 2018 to December 2019 in the jurisdiction of the primary medical institutions in Yinchuan city,among which 180 patients who had high blood pressure,diabetes,coronary heart disease,COPD and other chronic diseases were as the observation group and the other 180 patients with the symptoms but not as chronical diseases as the control group.All the patients would wear the single lead wearable ECG device to collect ECG data for 24 hours and upload the data to the cloud in real time when they had the above symptoms.The primary doctors made a preliminary diagnosis based on the scatter diagram and then the final diagnosis reports were issued by the Remote ECG Diagnosis Center of Yinchuan First People's Hospital.The detection rate of arrhythmia and the type and number of arrhythmia detected between the two groups were compared.Results The total detection rate of arrhythmia was 68.61%and the observation group was 89.44%which was significant higher than that of the control group(47.8%)(P<0.05).Conclusion The single-lead wearable ECG device is practical and portable,which significantly improves the detection rate of arrhythmia and it is conducive to the management of patients with chronic diseases.
