摘要
目的:探讨品管圈结合SBAR沟通模式在交接班的应用效果。方法:成立品管圈小组,确立"缩短交接班时间"为活动主题,应用查检表对护士交接班时间现状进行调查,将2018年10至12月102次交班设对照组,将2019年1至3月105次交班设为实验组。对照组按照传统的方法由全体护士在护士站进行总交班,接班护士自行记录交班重点。实验组成立品管圈,组织全体护士学习SBAR标准化沟通模式,并应用于交接班中,由责任护士每班更新SBAR交接班表内容并电子版存档,接班护士持SBAR交班表进行交班。比较二者的交接班时间、病人和护士满意度、护士不良事件发生率、护士检查预约漏通知率和病人检查项目知晓率。结果:品管圈结合SBAR沟通模式实施后,实验组交接班时间比对照组明显缩短(P<0.05)。在保证护理质量的前提下,实验组护士满意度和病人检查项目知晓率均比对照组有提高(P<0.05)。同时,实验组的护士检查预约漏通知率、护士不良事件发生率有所下降。结论:开展品管圈活动结合SBAR标准化沟通工具,可以明显缩短护理交接班时间,减少不良事件发生,提高护士满意度。
Objective:To explore the application effect of quality control circle(QCC)combined with SBAR communication model in shift handover.Methods:QCC group was set up and the theme of the activity"shorten the time of handover"was established.The status of nurse handover time with checklist of 102 shifts from October to December in 2018 was as the experimental group.105 shift from January to March in 2019 was as control group.In the control group,the total shift was carried out by all the nurses in the nurse station according to the traditional method,and the key points of the shift were recorded by the successors themselves.In the experimental group,the product management circle was set up,and all the nurses were organized to learn the standardized communication mode of SBAR,and applied to the transfer class.The responsible nurses updated the SBAR shift content in each class and archived the electronic version,and the succession nurses held the SBAR shift form to carry on the shift.The shift time before and after implementation,patient and nurse satisfaction,nurse inspection missed implementation rate and adverse events incidence were compared.Results:After combination of QCC and SBAR communication mode,the shift handover time was significantly shortened(P<0.05).On the premise of ensuring the quality of nursing,nurses’satisfaction and patients’awareness of examination items were improved(P<0.05).At the same time,the missed execution rate and the incidence of adverse events decreased.Conclusion:Carrying out quality control circle activities combined with SBAR standardized communication tools can significantly shorten the nursing shift time,reduce adverse events,and improve the satisfaction of nurses.
