摘要
目的调查母乳α-生育酚和宏量营养素的含量及变化趋势,探索母体脂代谢相关因素对乳成分的可能影响方法招募上海地区单胎足月分娩的健康产妇89例,采集基础资料和血脂等生化检查数据,随访收集初乳、过渡乳和成熟乳测定α-生育酚和宏量营养素的含量,分析哺乳阶段和母体因素的影响结果随哺乳期进展,α-生育酚明显下降(初乳中9.72 mg/L,成熟乳中4.23 mg/L),同时母乳乳糖(初乳中7.54 mg/dL,成熟乳中6.95 mg/dL)及蛋白质(初乳中1.29 mg/dL,成熟乳中0.99 mg/dL)轻度下降,而脂肪剧增(初乳中1.58 mg/dL,成熟乳中3.23 mg/dL);各阶段母乳α-生育酚与脂肪含量均正相关;母体BMI与过渡乳的α-生育酚和脂肪含量正相关,母血总甘油三酯与过渡乳α-生育酚正相关,未发现孕期增重、年龄等母体因素对母乳成分有影响结论上海母乳α-生育酚含量明显偏高,与脂肪含量存在正相关关系,在过渡乳阶段受到母体长期脂代谢营养状况的影响。
Objective To investigate the concentrations of breast milk macro-nutrients andα-tocopherol and to explore possible associations with maternal lipids metabolismMethodsColostrum,transitional milk and mature milk were collected longitudinally from 89 healthy Shanghai lactating mothers of singleton,full-term delivery.The levels of macro-nutrients andα-tocopherol in human milk from different lactation stages were analyzed and compared.Perinatal medical history and blood biochemical data were also collected for correlation analysis.Results From colostrum to mature stage,breast milkα-tocopherol decreased sharply from 9.72 mg/L to 4.23 mg/L while lactose(from 7.54 mg/dL to 6.95 mg/dL)and protein decreased slightly(from 1.29 mg/dL to 0.99 mg/dL).However,fat content increased sharply from 1.58 mg/dL to 3.23 mg/dL.A positive correlation was found between milk fat andα-tocopherol concentrations during each lactation stage.Maternal BMI was positively correlated with the levels ofα-tocopherol and fat in transitional milk.Maternal blood triglyceride was positively correlated with the levels ofα-tocopherol in transitional milk.Meanwhile,no other correlations were observed.Conclusion Shanghai breast milk contained an abundant level ofα-tocopherol,showing a positive correlation with milk fat content.Maternal long-term lipids metabolism may have an influence over milk composition,especially transitional milk.
